THE APPLICANT REQUESTS THAT: 

 

1. He be entitled to the award of the Air Force Outstanding 
Unit Award (AFOUA). The AFOUA with One Bronze Oak Leaf Cluster (AFOUA, w/1BOLC was ADMINISTRATIVELY CORRECTED). 


 

2. He be entitled to the Air Force Organizational Excellence 
Award. 

 

3. In addition, he requests a copy of his DD Form 214, Certificate of Release or Discharge from Active Duty, reflecting 
his permanent disability retirement, with a compensable 
disability rating of 30 percent.

THE APPLICANT CONTENDS THAT: 

 

He served under several units, including units at the squadron, 
wing, air division and numbered Air Force, and major command 
levels from July 1986 thru January 1991, . 

 

He believes these decorations should have been awarded and 
listed on his DD Form 214 at the time of his release from active 
duty. 

 

In support of his appeal, the applicant provides a Unit Fact 
sheet; a copy of his DD-214, issued in conjunction with his 
4 Feb 1990 release from active duty; Special Orders (SO) AB-
3780, AA-4350; Statement of Service, and a letter of 
congratulation.

STATEMENT OF FACTS: 

 


Based on the available record, the applicant was relieved from 
active duty, effective 19 NOV 08 and placed on the Temporary 
Disability Retired List (TDRL) in the grade of master sergeant, 
per AFI 36-3212, Physical Evaluation For Retention, Retirement, 
And Separation, with a compensable rating for physical 
disability of 30 percent. On 30 Aug 10, he was removed from the 
TDRL and retired, with a compensable disability rating of 
30 percent for physical disability. He was credited with 
4 year, 7 months, and 11 days of active duty service for 
retirement. 

 

The remaining relevant facts pertaining to this application, 
extracted from the applicant’s military records, are contained 
in the letter prepared by the appropriate office of the Air 
Force.

THE AIR FORCE EVALUATION: 

 

AFPC/DPSIDR recommends denial stating, in part, that they were 
not able to verify the applicant’s entitlement to the AFOEA, 
without any documentation that he was directly assigned to the 
18th Aircraft Maintenance Squadron, the 313th Air Division, the 
Fifth Air Force and PACAF. 

 

They were able to verify the applicant's entitlement to the 
AFOUA with one Bronze Oak Leaf Cluster (AFOUA w/1BOLC) while he 
was assigned to the 18th Component Repair Squadron. The 18th 
Tactical Fighter Wing received this same award. The applicant's 
record has been forwarded to another office for an update. 

 

The AFOUA is awarded by the Secretary of the Air Force to 
numbered units that have distinguished themselves by 
exceptionally meritorious service or outstanding achievement 
that clearly sets the unit above and apart from similar units. 
The services include; performance of exceptionally meritorious 
service, accomplishment of a specific outstanding achievement of 
national or international significance, combat operations 
against an armed enemy of the United States, or military 
operations involving conflict with or exposure to hostile 
actions by an opposing foreign force. 

 

The AFOEA is awarded to Air Force internal organizations that 
are entities within larger organizations. They are unique, 
unnumbered organizations or activities that perform functions 
normally performed by numbered wings, groups, squadrons, etc.

APPLICANT'S REVIEW OF THE AIR FORCE EVALUATION: 

 


A copy of the Air Force evaluation was forwarded to the 
applicant on 22 April 2011 for review and comment within 
30 days. As of this date, no response has been received by this 
office (Exhibit C).

THE BOARD CONCLUDES THAT: 

 

1. The applicant has not exhausted all remedies provided by 
existing law or regulations in regard to his request for a 
DD Form 214, reflecting his permanent disability retirement. In 
this respect, we advise the applicant to contact his issuing 
command, who has the authority to issue or provide the DD Form 
214 upon release or discharge from active duty. In addition, we 
note, in accordance with the governing Department of Defense 
Instruction (DODI) and Air Force Instruction (AFI), a DD Form 
214 need not be prepared when a member is removed from the TDRL. 

 

2. The application was timely filed. 

 

3. Insufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice warranting 
corrective action in regard to the applicant’s entitlement to 
the AFOEA. We took notice of the applicant's complete 
submission in judging the merits of the case; however, we agree 
with the opinion and recommendation of the Air Force office of 
primary responsibility and adopt its rationale as the basis for 
our conclusion that relief beyond that already granted 
administratively is not warranted. Therefore, we find no basis 
to favorably consider this application.

THE BOARD DETERMINES THAT: 

 

The applicant be notified the evidence presented did not 
demonstrate the existence of material error or injustice; the 
application was denied without a personal appearance; and the 
application will only be reconsidered upon the submission of 
newly discovered relevant evidence not considered with this 
application.
