1. His SF 88, Report of Medical Examination, dated 24 November 
1987, be amended to reflect the following: 

 

 A. Item 24, Eyes – Abnormal rather than Normal. 

 

Add the following notes regarding Item 24: 

 

 24a. Defective distant vision acuity diagnosis 13 June 1967. 

 

 24b. 17 vision examinations and prescription eye glasses 
issued due to changes and corrections to member’s vision acuity. 

 

 24c. Defective distant visual acuity OU corrected 
24 November 1987. 

 

 24d. Defective near visual acuity OD inadequately corrected 
24 November 1987. 

 

 B. Item 35, Upper Extremities – Abnormal rather than 
Normal. 

 

Add the following notes regarding Item 35: 

 

 Left hand: fracture/crush injury 30 August 1978; 
injury/inflammation 3 November 1979; assessment-decreased 
strength, periodic Medical Examination 24 January 1980; injury to 
wrist/forearm 29 May 1981. 

 

 Right hand: fracture right hand 26 June 1978 and injury 
right wrist 23 April 1980. 

 

 C. Item 36, Feet – Abnormal rather than Normal.

2. His SF93, Report of Medical History, dated 24 November 1987, 
be amended in Item 25, Physician’s summary and elaboration of all 
pertinent data to reflect the notes from the SF 88, Report of 
Medical Examination.

APPLICANT CONTENDS THAT: 

 

He believes he was not properly processed for retirement by the 
medical department. The medical personnel at the USAF Regional 
Hospital gave him instructions and information that was contrary 
to what was required of SF 88, Report of Medical Examination and 
SF 93, Report of Medical History because when he declared his 
active duty injuries and his problems to them he was told that he 
did not have to worry about them because he would have continued 
medical care as a retiree by the military health care system and 
the Veteran’s Administration. 

 

In support of his request, the applicant provides a personal 
statement and medical documentation.

Exhibit A.

STATEMENT OF FACTS: 

 

On 31 July 1988, the applicant was relieved from active duty and 
retired on 1 August 1988, in the grade of master sergeant. He 
served 20 years, 8 months and 26 days on active duty.

AIR FORCE EVALUATION: 

 

AFMOA/SGAT recommends partial approval of the applicant’s 
request. SGAT states that due to the lack of dental clinic 
documentation, SGAT cannot recommend amending the SF 88 and SF 
93 to reflect any dental procedures performed.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

On 27 November 2010, a copy of the Air Force evaluation was 
forwarded to the applicant for review and comment within 30 days 
(Exhibit D). As of this date, this office has received no 
response.

AIR FORCE EVALUATION: 

 

AFMOA/SGAT states that it appears the applicant’s vision was 
diagnosed as defective at some point and is noted as such in the 
24 November 1987 visit in section 74 on page 2. However, if this 
diagnosis cannot be found in the current record, then SGAT 
recommends adding it to the 24 November 1987 visit to give 
validation to the doctor’s notes.

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by existing 
law or regulations. 

 

2. The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice warranting 
partial relief. We note that AFMOA/SGAT recommends approval of 
the applicant’s request with the exception of the dental portion 
due to lack of dental clinic documentation. We agree with their 
assessment and recommend his records be corrected to the extent 
indicated below.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air Force 
relating to APPLICANT, be corrected to show that: 

 

1. His Standard Form 88, Report of Medical Examination, dated 
24 November 1987, be amended to reflect the following:
