THE APPLICANT REQUESTS THAT: 

 

His record be corrected to show that he was awarded the Air 
Force Commendation Medal, Second Oak Leaf Cluster (AFMC, 
w/2nd OLC) and his Home of Record (HOR) be changed to Memphis TN 
(Administratively Corrected).

THE APPLICANT CONTENDS THAT: 

 

His DD Form 214, Certificate of Release or Discharge from Active 
Duty, issued in conjunction with his 12 Apr 83 discharge only 
reflected his AFCM, w/1OLC, not with the 2OLCs he received at 
discharge. 

 

He enlisted in Memphis, Tennessee (TN) as noted in his 
DD Form 214, Certificate of Release or Discharge from Active 
Duty, issued in conjunction with his 12 Aug 76 discharge. 

 

In support of his appeal, the applicant provides a copy of his 
DD Forms 214, (12 Aug 76) and (12 Apr 83); a copy of citation 
and certificate for award of the AFCM, w/1OLC and a copy of the 
citation for the award of the AFCM, w/2OLCs.

Exhibit A.

STATEMENT OF FACTS: 

 

The relevant facts pertaining to this application, extracted 
from the applicant’s military records, are contained in the 
letter prepared by the appropriate office of the Air Force. 

 

The applicant’s HOR will be administratively corrected to 
reflect his HOR as Memphis TN.

THE AIR FORCE EVALUATION: 

 

AFPC/DPSIDRA recommends denial, stating, in part, a copy of the 
AFCM w/2OLCs certificate and special order was not located in 
the applicant’s military record nor were they provided. A copy 
of the special order was requested; however, the applicant did 
not respond. Without a copy of the special order, they are not 
able to verify the applicant’s entitlement to the AFCM w/2OLCs.

APPLICANT'S REVIEW OF THE AIR FORCE EVALUATION: 

 

A copy of the Air Force evaluation was forwarded to the 
applicant on 17 Sep 10 for review and comment within 30 days. 
As of this date, no response has been received by this office 
(Exhibit D).

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by 
existing law or regulations. 

 

2. The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice warranting 
award of an additional Oak Leaf Cluster (OLC) to the Air Force 
Commendation Medal (AFCM). We note the comments by AFPC/DPSIDR 
that they were not able to verify the applicant’s entitlement to 
the requested award. Nonetheless, after a careful review of the 
available record and the applicant’s submission, including the 
citation for the award of the AFCM, 2nd OLC, we are convinced of 
the applicant’s entitlement to the requested award and that it 
was awarded at the time of his separation. Therefore, to 
preclude any further injustice to the applicant, we recommend 
his record be corrected to the extent indicated below.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air 
Force relating to APPLICANT, be corrected to show that his 
DD Form 214, Certificate of Release or Discharge from Active 


Duty, issued in conjunction with his 12 April 1983 discharge, be 
amended in Block 13, Decorations, Medals, Badges, Citations and 
Campaign Ribbons Awarded or Authorized, to reflect “Air Force 
Commendation Medal, with Two Oak Leaf Clusters,” rather than 
“Air Force Commendation Medal, with One Oak Leaf Cluster.”
