THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of his second Army Commendation Medal (ARCOM).

2.  The applicant states this award of the ARCOM was not processed prior to his release from active duty (REFRAD).

3.  The applicant provides copies of:

* DD Form 214 (Member copies Number 1 and 4)
* DA Form 4980 (Certificate for award of ARCOM) with Permanent Orders Information, dated 16 August 2011
* DA Form 4980 with Permanent Orders Information, dated 17 March 2014
* Enlisted Record Brief (ERB) dated 20 May 2014

CONSIDERATION OF EVIDENCE:

1.  On 27 January 2010, the applicant enlisted in the Regular Army and was trained as an M1 armor crewman.

2.  A DA Form 4980, as provided by the applicant, shows that Permanent Orders Number 228-067, dated 16 August 2011, announced his award of the ARCOM for meritorious service during the period 12 February 2011 to 31 January 2012.

3.  A DA Form 4980, as provided by the applicant and filed in his Official Military Personnel File (OMPF), shows that Permanent Orders Number 076-04, dated 
17 March 2014, announced his award of the ARCOM for meritorious service during the period 27 January 2010 to 25 May 2014.

4.  The applicantÂ’s ERB shows he was twice awarded the ARCOM.

5.  The applicant's DD Form 214 reports that:

	a.  On 25 May 2014, he was REFRAD and transferred to the 352nd Transportation Detachment.  He had attained the rank of specialist, pay 
grade E-4 and had completed 4 years, 3 months, and 29 days of creditable active service.

	b.  Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) indicates he was awarded one ARCOM.

6.  Army Regulation 635-8 (Separation Processing and Documents) provides detailed instructions for completing separation documents, including the DD Form 214. The DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge.  All awards for all periods of service are to be entered on the DD Form 214.

DISCUSSION AND CONCLUSIONS:

1.  The applicant contends that his DD Form 214 should be corrected to show two awards of the ARCOM.

2.  The available evidence shows that the applicant was twice awarded the ARCOM for meritorious service.  Therefore, his DD Form 214 should be corrected to reflect both awards.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting from his DD Form 214 the Army Commendation Medal; and

	b.  adding to his DD Form 214 the Army Commendation Medal (two awards).
