THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests removal of the entry in item 14 (Military Education) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 20 June 2011.

2.  The applicant states, prior to completion of the Infantry Officer Advanced Course, he received a general officer letter of reprimand (GOMOR) and he was dismissed from the course for disciplinary reasons.

3.  The applicant provides:

* the GOMOR, dated 16 April 2009
* his rebuttal, dated 5 May 2009
* the GOMOR filing determination, dated 14 May 2009
* his DD Form 214

CONSIDERATION OF EVIDENCE:

1.  Upon completion of Officer Candidate School, the applicant was appointed a Reserve Commissioned Officer of the Army on 5 December 2002, in the Infantry Corps.

2.  On 4 June 2007, he was ordered to active duty to fulfill his 3-year active duty commitment with a reporting date to Fort Riley, KS, effective 15 September 2007.


3.  The applicant's record shows he received a GOMOR on 16 April 2009 for driving under the influence while attending the Maneuver Captains Career Course at Fort Benning, GA.

4.  The applicant provides and his record contains a memorandum signed by the Commanding General, Headquarters, U.S. Army Infantry Center, Fort Benning, GA, dated 14 May 2009, that directed the applicant's GOMOR be filed in his Official Military Personnel File (OMPF), now known as the Army Military Human Resources Record (AMHRR).

5.  On 20 June 2011, the applicant was retired by reason of temporary physical disability and placed on the Temporary Disability Retired Listed (TDRL) in the rank/grade of captain (CPT)/O-3.  He completed 3 years, 9 months, and 6 days of net active service this period.  The DD Form 214 he was issued at the time shows in item 14 the entry "INF OFF (Infantry Officer) ADVANCED, 20 WEEKS, 2009//NOTHING FOLLOWS."

6.  There is no evidence in the applicant's record that shows he ever completed the Infantry Officer Advanced Course.  His record contains an Officer Record Brief (ORB) prepared on 6 April 2009.  Section VI (Military Education) shows completion of the Infantry Officer Basic Course in 2003.  It does not show completion of the Infantry Officer Advanced Course.

7.  Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214.  The purpose of the separation document is to provide the individual with documentary evidence of his or her military service.  It is important that information entered on the form should be complete and accurate.  Chapter 2 contains guidance on the preparation of the DD Form 214 and states for item 14, enter the formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 and include title, length in weeks, and year completed.

DISCUSSION AND CONCLUSIONS:

There is no evidence the applicant every completed the Infantry Officer Advanced Course.  In fact, the applicant states he was dismissed from the course for disciplinary reasons.  It appears the current entry in item 14 of his DD form 214 is the result of an administrative error.  Therefore, his DD Form 214 for the period ending 20 June 2011 should be corrected as recommended below.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting from item 14 of his DD Form 214 for the period ending 20 June 2011 and replacing it with the entry "NONE."
