Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of item 12d (Total Prior Active Service) of his DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states he served on active duty from 17 May 2005 to 30 July 2005. 3. The applicant provides: * DD Form 220 (Active Duty Report) * DD Form 214 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the U.S. Army Reserve (USAR) on 17 September 2004 for 8 years. 2. He provided a DD Form 220, dated 9 July 2005, which shows he entered active duty on 17 May 2005 and departed active duty on 30 July 2005, a total of 2 months and 14 days. 3. He enlisted in the Regular Army on 21 March 2006 for 5 years. On 17 August 2012, he was honorably discharged for entry in the Officer Training Program. 4. Item 12d of his DD Form 214 shows the entry "0000  00  00." 5. He was appointed as a second lieutenant in the USAR on 16 May 2014. 6. Army Regulation 635-5 (Separation Documents), in effect at the time, prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It established standardized policy for preparation of the DD Form 214. It stated to enter from previously issued DD Forms 214 the total amount of prior active military service less lost time in item 12d. 7. Army Regulation 635-8 (Separation Processing and Documents), currently in effect, prescribes policy and procedural guidance relating to transition management. It states to enter from previously issued DD Forms 214, DD Forms 220, DA Forms 1506 (Statement of Service – for Computation of Length of Service for Pay Purposes), or Retirement Point Annual Statements, the total amount of prior active military service less lost time in item 12d. DISCUSSION AND CONCLUSIONS: The DD Form 220 provided by the applicant which shows he served on active duty from 17 May 2005 to 30 July 2005 is accepted as sufficient evidence on which to amend item 12d of his DD Form 214 to show this prior active service.
