THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he entered active duty on 14 February 2012 instead of 14 February 2010. 2. He states the date recorded on his DD Form 214 is in error because this is not the date he entered the service. 3. He provides no additional documents. CONSIDERATION OF EVIDENCE: 1. The applicant's DD Form 4 (Enlistment/Reenlistment Document - Armed Forces of the United States) shows he enlisted in the U.S. Army Reserve under the delayed entry program on 25 July 2011 and he enlisted in the Regular Army on 14 February 2012. 2. His Enlisted Record Brief shows his pay entry basic date and basic active service date as 14 February 2012. 3. He was retired from active duty on 23 April 2013 by reason of physical disability. 4. He was issued a DD Form 214 which shows he entered active duty on 14 February 2010 and was medically retired on 23 April 2013. This is 3 years, 2 months and 9 days of active military service. This DD Form 214 indicates he completed 1 year, 2 months, and 10 days of active duty service. 5. Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214. It states that the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement, or discharge. It directs that the beginning date of the continuous period of active duty will be entered in item 12a (Date Entered Active Duty This Period) for issuance of a DD Form 214 for which a DD Form 214 was not previously issued. DISCUSSION AND CONCLUSIONS: 1. The applicant's enlistment contract confirms he entered active duty on 14 February 2012. However, his DD Form 214 reflects he entered this period of active duty on 14 February 2010. 2. The applicant was medically retired on 23 April 2013 and he completed 1 year, 2 months, and 20 days of active duty this period. It appears the applicant's date of entry on active duty for this period is erroneously entered in item 12a of his DD Form 214. 3. Therefore, it would be appropriate to amend item 12a of the applicant's DD Form 214 to show he entered active duty on 14 February 2012.
