THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of block 7a (Place of Entry into Active Duty) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show Boston, Massachusetts instead of Boston, Michigan. 2. The applicant states the incorrect state was recorded on his DD Form 214. 3. The applicant provides a DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the U.S. Army Reserve Delayed Entry Program on 15 October 2009. His DD Form 4 (Enlistment/Reenlistment Document) shows in block 3 (Home of Record), Plymouth, Massachusetts, and in block 4 (Place of Enlistment/Reenlistment), Boston, Massachusetts. 2. He enlisted in the Regular Army on 20 January 2010 and he was honorably released from active duty on 8 June 2013. His DD Form 214 shows in block 7a the entry "Boston, Michigan." 3. Army Regulation 635-5 (Separations Documents), in effect during the applicant's active duty service, prescribed the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It established standardized policy for the preparation of the DD Form 214. It stated the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active service. The regulation provided that a Soldier's initial enlistment contract or appointment document is the source for the data entered in block 7a. DISCUSSION AND CONCLUSIONS: 1. The applicant's DD Form 4 clearly shows his home of record and place of enlistment as Massachusetts. It appears that during the preparation of his DD Form 214 an administrative error occurred that resulted in the recording of the State of Michigan as his place of entry into active duty instead of the State of Massachusetts. 2. In view of the foregoing, his DD Form 214 should be corrected to show the entry "Boston, Massachusetts" in block 7a.
