Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his fourth award of the Army Commendation Medal (ARCOM 4th Award) be added to his DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states he received his fourth award of the ARCOM subsequent to his release from active duty (REFRAD) and it has not been added to his DD Form 214. 3. The applicant provides copies of all four awards of the ARCOM and his DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 17 September 2007. He completed his training and was assigned to Fort Hood, Texas and then transferred to Fort Carson, Colorado with his unit. 2. On 16 October 2011, he was honorably REFRAD. His DD Form 214 shows he was awarded the Afghanistan Campaign Medal with campaign star, Iraq Campaign Medal with two campaign stars, Army Commendation Medal (3rd Award), Purple Heart, NATO Medal, Army Good Conduct Medal, National Defense Service Medal, Global War on Terrorism Service Medal, Army Service Ribbon, Overseas Service Ribbon (2nd Award), and the Combat Medical Badge. 3. The applicant’s fourth award of the ARCOM was awarded in Permanent Orders Number 349-001 issued by Headquarters, 1st Brigade Combat Team on 15 December 2011, 2 months after his REFRAD. DISCUSSION AND CONCLUSIONS: The applicant was awarded a fourth award of the ARCOM after his REFRAD and is entitled to have that award added to his DD Form 214.
