THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his last DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he was awarded the Army Achievement Medal. 2. The applicant states his last DD Form 214 indicates the Air Force Achievement Medal; it should be an Army Achievement Medal, not Air Force. His final discharge DD Form 214 from the U.S. Air Force (USAF) does not indicate he was awarded the Air Force Achievement Medal. He received the Army Achievement Medal on 1 December 2012. 3. The applicant provides DD Forms 214 dated 12 December 1989 and 17 November 2010. CONSIDERATION OF EVIDENCE: 1. The applicant served in the Regular Air Force from 11 August 1983 until he was honorably discharged on 12 December 1989. His DD Form 214 for the period ending 12 December 1989 does not show the Air Force Achievement Medal. 2. On 9 August 2008, the applicant enlisted in the Maryland Army National Guard (MDARNG). 3. The applicant was ordered to active duty for training (ADT) on 12 October 2010. He was released from ADT on 17 November 2010 to accept an appointment in the MDARNG in the rank of warrant officer one (WO-1). His DD Form 214 for the period ending 17 November 2010 shows, in part, the Air Force Achievement Medal. 4. On 18 November 2010, the applicant accepted an appointment in the MDARNG in the rank of WO-1. 5. The applicant was ordered to ADT on 29 October 2011. He was released from ADT at the completion of his required active service on 3 July 2012. The DD Form 214 he received does not show he was awarded the Army Achievement Medal but does show he was awarded the Air Force Achievement Medal. 6. On 1 December 2012, the applicant was awarded the Army Achievement Medal for the period 26 through 30 November 2012. 7. Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army. It establishes standardized policy for preparing and distributing the DD Form 214. It states the DD Form 214 is a synopsis of the Soldier’s most recent period of continuous active duty. It states that item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) will contain all authorized awards, as contained in the Army's awards regulation, for all periods of service. DISCUSSION AND CONCLUSIONS: 1. The applicant's contentions have been noted. His supporting evidence has been considered. 2. The applicant was not awarded the Army Achievement Medal until 1 December 2012. The DD Form 214 that he is requesting to have corrected covered his period of active duty service from 29 October 2011 through 3 July 2012. The Army Achievement Medal is properly not shown on that DD Form 214 because it was awarded for a later period of service. 3. In view of the foregoing, the applicant's request should be denied. 4. It was not clear if the applicant was requesting that the Air Force Achievement Medal be deleted from his DD Forms 214 for the periods ending 17 November 2010 and 3 July 2012. Therefore, no action will be taken regarding that award at this time.
