THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that her DD Form 214 (Certificate of Release or Discharge from Active Duty), dated 21 December 2012, be corrected to show: * she entered active duty in 2009 * award of the Army Achievement Medal (AAM) 2. The applicant states she was mobilized 1 year stateside to train in a new functional area prior to deploying to Afghanistan; however, that period of service is not reflected on her DD Form 214. She is attempting to gather all of her orders, but she has her leave and earnings statements (LES) to show her service. 3. The applicant provides copies of orders, LESs, and a DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant’s LESs show that she was ordered to active duty training (ADT) on 19 October 2009 and she was paid for continuous ADT up to and including 5 November 2010, the date her DD Form 214 reflects she was ordered to active duty in support of Operation Enduring Freedom. 2. She deployed to Afghanistan from 19 December 2010 through 25 October 2011 and she was honorably released from active duty (REFRAD) on 21 December 2011. Her DD Form 214 shows that she entered active duty on 5 November 2010 and that she served 1 year, 1 month, and 17 days of active service this period. 3. The AAM Certificate provided by the applicant shows she was awarded the AAM on 12 May 2012 for the period 12 March to 13 March 2012. 4. A review of her official records failed to show any evidence of a DD Form 214 being issued for the period of ADT from 19 October 2009 to 4 November 2010, a period of 1 year and 15 days. The applicant was transferred to the Retired Reserve in September 2013 due to maximum years of service. 5. Army Regulation 635-5 (Separation Documents) serves as the authority for the preparation of the DD Form 214. It provides, in pertinent part, that only awards that are earned during the period covered by the DD Form 214 or earlier maybe entered on the DD Form 214. Awards earned after the date of the DD Form 214 are not authorized for retroactive entry on the DD Form 214 unless the period of the award earned is within the period covered by the DD Form 214. DISCUSSION AND CONCLUSIONS: 1. While it appears that the applicant should have been issued a DD Form 214 for ADT for the period 19 October 2009 to 4 November 2010, there is no evidence to show that such was accomplished. 2. Therefore, since she was on continuous active duty during the period 19 October 2009 to 21 December 2011, her last DD Form 214 should be corrected to show in item 12a that she entered active duty this period on 19 October 2009. 3. Additionally, item 12c should be corrected to show that she served 2 years, 2 months, and 3 days of active service this period. 4. However, her request to add the AAM to her DD Form 214 appears to lack merit. The AAM was earned after the period covered by her last DD Form 214 and thus is not authorized for retroactive entry on her DD Form 214. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: * Deleting the entry “2010 11 05” from item 12a of her DD Form 214, dated 21 December 2011 * Deleting the entry “0001 01 17” from item 12c of her DD Form 214 * Entering the entry “2009 10 09” in item 12a of her DD Form 214 * Entering the entry “0002 02 03” to item 12c of her DD Form 214 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
