Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the North Atlantic Treaty Organization (NATO) Medal. 2. The applicant states the NATO Medal was not added to his DD Form 214. 3. The applicant provides his DD Form 214 and a certificate awarding him the NATO Medal. CONSIDERATION OF EVIDENCE: 1. The applicant's records show he was appointed as a Reserve commissioned officer of the New York Army National Guard (NYARNG) and executed an oath of office on 21 April 2010. 2. He completed the Army Medical Department Basic Officer Leader Course from 12 August to 6 September 2011. He was assigned to C Company, 427th Brigade Support Battalion. 3. He was ordered to active duty on 28 January 2012 and subsequently served in Afghanistan from 13 April 2012 to 15 July 2012 - a period of 3 months and 3 days. He was honorably released from active duty on 2 August 2012. His DD Form 214 shows in: * Item 12f (Foreign Service) - the entry "0000-00-00" * Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) - he was awarded or authorized the: * Afghanistan Campaign Medal with bronze service star * National Defense Service Medal * Global War on Terrorism Service Medal * Army Service Ribbon * Overseas Service Ribbon * Armed Forces Reserve Medal with "M" Device 4. Permanent Orders 197-08, issued by Headquarters, Task Force Medical Afghanistan on 16 July 2012, awarded him the Army Commendation Medal for service from 4 April to 13 July 2012 in Afghanistan. 5. His records contain a certificate showing he was awarded the NATO Medal for service with NATO in Afghanistan from 13 April 2012 to 15 September 2012. 6. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The purpose of the separation document is to provide the individual with documentary evidence of their military service. Item 12f is obtained from the Enlisted/Officer Record Brief and shows the total amount of foreign service completed during the period covered in by the DD Form 214 and item 13 shows all awards and decorations. DISCUSSION AND CONCLUSIONS: 1. The applicant served in Afghanistan from 13 April 2012 to 15 July 2012, a period of 3 months and 3 days. Therefore, he is entitled to correction of his DD Form 214 to show his foreign service in item 12f. 2. He was awarded the Army Commendation Medal and the NATO Medal but neither is shown on his DD Form 214. Therefore, he is entitled to correction of his DD Form 214 to show these awards.
