Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 22 June 2012 to show her foreign service in Iraq from 8 May 2003 to 19 March 2004. 2. The applicant states her foreign service in Iraq is shown on her DD Form 214 for the period ending 24 May 2004 but it is not shown on her DD Form 214 for the period ending 22 June 2012. 3. The applicant provides two DD Forms 214 and a DD Form 214 Worksheet. CONSIDERATION OF EVIDENCE: 1. The applicant's records show she enlisted in the U.S. Army Reserve (USAR) on 17 May 1997 and she held military occupational specialties 91X (Mental Health Specialist) and 91W (Medical Specialist). 2. She was ordered to active duty as a member of her USAR unit in support of Operation Enduring/Iraqi Freedom and she entered active duty on 7 February 2003. 3. She served in Kuwait/Iraq from 17 April 2003 to 2 April 2004 while assigned to the 113th Medical Combat Stress Control Company. 4. She was honorably released from active duty on 24 May 2004. She completed 1 year, 3 months, and 18 days of creditable active service during this period of service. 5. The DD Form 214 she was issued for this period of service contains the following entries in: * item 12f (Foreign Service) "0001--00--00" * item 18 (Remarks) "SERVICE IN KUWAIT FROM 20030417-20030508 AND 20040319-20040402" and "SERVICE IN IRAQ FROM 20030508-20040319" 6. She was ordered to active duty as a member of her USAR unit in support of Operation Enduring Freedom and she entered active duty on 29 May 2011. 7. She served in Afghanistan from 2 July 2011 to 13 May 2012 while assigned to the 113th Medical Company (Combat Stress Control). 8. She was honorably released from active duty on 22 June 2012. She completed 1 year and 24 days of creditable active service during this period of service. 9. The DD Form 214 she was issued for this period of service contains the following entries in: * item 12f "0000--10--12" * item 18 "SERVICE IN AFGHANISTAN FROM 20110702-20120513" 10. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states in item 12f, enter the total amount of foreign service completed during the period covered by the DD Form 214 and in item 18, for a Reserve Soldier ordered to active duty and deployed to a foreign country, enter the following three statements in succession: * “ORDERED TO ACTIVE DUTY IN SUPPORT OF (OPERATION NAME) PER 10 USC (applicable section)” * “SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)” * “SOLDIER HAS/HAS NOT COMPLETED PERIOD FOR WHICH ORDERED TO ACTIVE DUTY FOR PURPOSE OF POST-SERVICE BENEFITS AND ENTITLEMENTS" DISCUSSION AND CONCLUSIONS: The regulatory guidance states the foreign service completed during the period covered by the DD Form 214 is listed on the DD Form 214. Her DD Form 214 for the period ending 22 June 2012 correctly shows her foreign service in Afghanistan that was completed during the period covered by the DD Form 214. There are no provisions for adding foreign service completed during a prior period of active duty to the DD Form 214. Therefore, she is not entitled to the requested relief.
