Army Regulation 600-8-22 (Military Awards) provides that the Army Achievement Medal is awarded to members of the Armed Forces of the United States who, while serving in a noncombat area on or after 1 August 1981, distinguished themselves by meritorious service or achievement. As with all personal decorations, formal recommendations, approval through the chain of command, and announcement in orders are required. DISCUSSION AND CONCLUSIONS: 1. With respect to the Army Commendation Medal, permanent orders awarded him an Army Commendation Medal which is not shown on his DD Form 214. Therefore, he is entitled to correction of DD Form 214 to show it. 2. With respect to the Army Achievement Medal: a. A permanent order awarded him an Army Achievement Medal which is already shown on his DD Form 214. b. He submitted an incomplete DA Form 638 with no permanent order or issuing headquarters that shows he was approved for award of a second Army Achievement Medal. The governing Army regulation states that for personal decorations, formal recommendation, approval through the chain of command, and announcement in orders are required. In the absence of orders, there is insufficient evidence to grant relief.
