Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests reconsideration of his request to add the Army Commendation Medal (ARCOM) to his DD Form 214 (Certificate of Release or Discharge from Active Duty) 2. The applicant states he wants his DD Form 214 updated or corrected to show the ARCOM. 3. The applicant provides: * Self-authored statement * ARCOM Certificate * five (5) complete DA Forms 638 (Recommendation for Award) * Memorandum for Record * three (3) Certificates of training * Assignment Order * two (2) NGB Forms 22 (Department of the Army and Air Force National Guard Bureau Report of Separation and Record of Service) * DA Form 2-1 (Personnel Qualification Record) CONSIDERATION OF EVIDENCE: 1. Incorporated herein by reference are military records which were summarized in the previous consideration of the applicant's case by the Army Board for Correction of Military Records (ABCMR) in Docket Number AR20100011918 on 20 October 2010. 2. The applicant provided a copy of a complete DA Form 638, dated 17 January 1997, and the certificate awarding him the Army Commendation Medal. These documents are new evidence that were not previously reviewed and warrant consideration by Board. 3. During the original deliberations in this case, the Board concluded: a. There was no evidence of the applicant's ARCOM in his military record. b. The applicant submitted the front side of the DA Form 638 recommending his award of the ARCOM; however, he failed to submit the reverse side showing the approval of the ARCOM and required orders data. c. Although the applicant submitted a copy of his DA Form 2-1 which included a handwritten entry of the ARCOM, the official DA Form 2-1 maintained in his Official Military Personnel File (OMPF) did not contain this entry. 4. Based on the information above, the Board determined there was an insufficient basis upon which to add the ARCOM to his DD Form 214. 5. The applicant now provides a copy of the complete DA Form 638 awarding him the ARCOM with corresponding certificate. These documents show the applicant was awarded the ARCOM for exceptional meritorious service during the period 4 January 1993 to 21 February 1997, by Permanent Orders Number 064-01, issued by Headquarters, Fort Myer, Arlington, VA, dated 5 March 1997. DISCUSSION AND CONCLUSIONS: The evidence confirms the applicant was awarded the ARCOM and this award was announced in official orders by the appropriate award authority. Therefore, this award should be added to his DD Form 214 at this time.
