THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Commendation Medal and the Iraq Campaign Medal. 2. The applicant states these two awards are not listed on his DD Form 214. 3. The applicant provides his DD Form 214. CONSIDERATION OF EVIDENCE: 1. He enlisted in the Regular Army on 29 September 2005 for a period of 4 years and 24 weeks. He completed basic combat and advanced individual training and was awarded military occupational specialty 92F (Petroleum Supply Specialist). 2. On 8 July 2010, a physical evaluation board (PEB) found him physically unfit for duty. The PEB recommended a combined disability percentage of 50 percent and permanent disability retirement. 3. A certificate in his official military personnel file (OMPF) shows Headquarters, 260th Quartermaster Battalion, Permanent Order Number 200-01, dated 19 July 2010, awarded him the Army Commendation Medal for meritorious service during the period 13 December 2005 to 11 April 2010. 4. On 6 October 2010, he was honorably retired by reason of permanent disability and he was placed on the Retired List the following day. He completed 5 years and 8 days of creditable active service. His DD Form 214 shows in: a. Item 13 (Decorations, Medals, Badges, Commendations, Citations and Campaign Ribbons Awarded or Authorized) the: * Army Achievement Medal * Army Good Conduct Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Korea Defense Service Medal * Army Service Ribbon * Overseas Service Ribbon (2nd Award) b. Item 18 (Remarks) he served in Iraq from 20071015 - 20081015 (15 October 2007 through 15 October 2008). 5. Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom (OIF). The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of OIF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are: * Liberation of Iraq (19 March 2003 - 1 May 2003) * Transition of Iraq (2 May 2003 - 28 June 2004) * Iraqi Governance (29 June 2004 - 15 December 2005) * National Resolution (16 December 2005 - 9 January 2007) * Iraqi Surge (10 January 2007 - 31 December 2008) * Iraqi Sovereignty (1 January 2009 - date to be determined) DISCUSSION AND CONCLUSIONS: 1. Headquarters, 260th Quartermaster Battalion, Permanent Order Number 200-01, dated 19 July 2010, awarded him the Army Commendation Medal. Therefore, his DD Form 214 should be corrected to show this award. 2. His DD Form 214 shows he served in Iraq from 15 October 2007 through 15 October 2008 during one campaign phase. Therefore, he is entitled to the Iraq Campaign Medal with one bronze service star and correction of his DD Form 214 to show this award.
