Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) with a separation date of 30 September 2010 be corrected to show award of the Defense Superior Service Medal and completion of Airborne Training. He requests that the document awarding him the Defense Superior Service Medal be inserted in the performance section of his official military personnel file (OMPF). 2. The applicant states the above items were not included on his DD Form 214. 3. The applicant provides: * his DD Form 214 * orders, certificate, and citation awarding him the Defense Superior Service Medal * orders awarding him the Parachutist Badge * his Officer Record Brief (ORB), dated 23 September 2010 CONSIDERATION OF EVIDENCE: 1. On 30 September 2010, he retired, and he was placed on the Retired List in the rank of colonel/pay grade O6 the following day. He completed 26 years, 8 months, and 23 days of active service. 2. Item 13 (Decorations, Medals, Badges, Commendations, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 does not show he was awarded the Defense Superior Service Medal. 3. Item 14 (Military Education) does not show he completed Airborne Training. 4. Department of Defense, Washington Headquarters Services Military Personnel Awards Order Number 10-177, dated 31 August 2010, awarded him the Defense Superior Service Medal. 5. 1st Special Operations Command (Airborne), Fort Bragg, NC Orders 77-1, dated 28 June 1984, lists the applicant as having been awarded the Parachutist Badge for successful completion of Airborne Training from 22 June to 5 July 1984. 6. Section VI - Military Education of his ORB indicates he completed Airborne Training in 1984. 7. The orders awarding the applicant the Defense Superior Service Medal are not in the performance section of his official military personnel file (OMPF) on iPERMS (integrated Personnel Electronic Records Management System). However, a copy of the certificate and citation for this award are in the performance section of his OMPF in iPERMS. 8. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. a. All decorations, service medals, campaign credits, and badges awarded or authorized will be entered in item 13. b. All formal in-service (full-time attendance) training courses successfully completed during the period of service covered will be entered in item 14. DISCUSSION AND CONCLUSIONS: 1. Orders show he was awarded the Defense Superior Service Medal. Therefore, this award should be added to his DD Form 214. A copy of these orders should also be placed in the performance section of his OMPF on iPERMS. 2. Orders awarding the applicant the Parachutist Badge and his ORB show he completed Airborne Training in 1984. Therefore, this course should be added to his DD Form 214.
