THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his service on his DD Form 214 (Certificate of Release or Discharge from Active Duty), dated 6 February 2000, be corrected and that his subsequent DD Form 214 be corrected accordingly. 2. The applicant states his DD Form 214, dated 6 February 2000, incorrectly reflects: a. in block 11 (Military Occupational Specialty (MOS)) the amount of time he served in this MOS; and b. in block 12a (date entered active duty this period) as 7 December 1993 instead of 7 July 1993. 3. The above mistakes were carried over to his subsequent DD Form 214, dated 16 June 2008. Block 12d (Total Prior Active Service) reflects he served 6 years and 2 months instead of 6 years and 7 months. 4. The applicant provides copies of his DD Forms 214, a statement of service, and a copy of his enlistment contract. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 7 July 1993 for a period of 6 years and training in MOS 67R (Attack Helicopter Repairer). He completed advanced individual training at Fort Eustis, Virginia and was awarded MOS 67R on 20 January 1994. 2. He continued to serve through a series of three separate extensions of his enlistment for a total of an additional 7 months and was honorably released from active duty (REFRAD) on 6 February 2000 due to completion of required service. 3. His DD Form 214 issued at the time of his REFRAD shows in block 11 that he served 6 years and 1 month in MOS 67R. Block 12a shows he entered active duty on 7 December 1993 and that he served 6 years and 7 months of active service this period. 4. On 17 April 2008, he again enlisted in the Regular Army for a period of 3 years under the Warrant Officer Flight Training Program. 5. On 16 June 2008, he was honorably discharged to accept a commission or warrant in the Army. The DD Form 214 he was issued at the time shows in block 12d 6 years and 2 months of total prior active service. 6. Army Regulation 635-5 serves as the authority for the preparation of the DD Form 214. It provides, in pertinent part, that information entered on the DD Form 214 will be obtained from official records in effect at the time of separation. DISCUSSION AND CONCLUSIONS: 1. The evidence of record clearly shows the applicant was awarded MOS 67R on 20 January 1994 and he held this MOS until he was REFRAD on 6 February 2000, which was 6 years and 1 month. Accordingly, this entry is correct and there is no basis for changing block 11. 2. However, his DD Form 214 incorrectly reflects that he entered active duty this period on 7 December 1993 when in fact he entered active duty on 7 July 1993 (1993 07 07). Accordingly, his DD Form 214, dated 6 February 2000, should be corrected to reflect in block 12a "1993 07 07." 3. The applicant’s subsequent DD Form 214 issued on 16 June 2008 reflects in block 12d that he had served 6 years and 2 months of prior active service when in fact, he had served 6 years and 7 months of prior active service. Accordingly the DD Form 214 should be corrected. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. deleting the entry in block 12a of the DD Form 214, dated 6 February 2000, and adding "1993 07 07" (7 July 1993). b. deleting the entry in block 12d of the DD Form 214, dated 16 June 2008, and adding 6 years and 7 months of prior active service. 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
