THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 23 April 2010 to show: * awards of the Army Commendation Medal, Army Achievement Medal, and Combat Action Badge * completion of the Counter Improvised Explosive Device Training Level III Course and the MaxxxPro Plus OPNET [Operation/Operator Maintenance New Equipment Training] Course 2. The applicant states he did not have the paperwork upon his release from active duty. 3. The applicant provides: * orders for the requested awards * certificates for the requested training CONSIDERATION OF EVIDENCE: 1. The applicant's records show he enlisted in the Colorado Army National Guard (COARNG) on 18 July 2008. 2. He entered active duty for training (ADT) on 22 October 2008, completed basic combat and advanced individual training, and he was awarded military occupational specialty 13P (MLRS [Multiple-Launch Rocket System]/Fire Direction Specialist). He was honorably released from ADT on 18 March 2009 by reason of completion of his required active service. 3. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 for the period ending 18 March 2009 shows the National Defense Service Medal and the Army Service Ribbon. 4. On 15 April 2009, he was ordered to active duty in support of Operation Iraqi Freedom. He subsequently served in Iraq from 28 June 2009 to 26 March 2010. He was assigned to Headquarters and Headquarters Battery, 2nd Battalion, 157th Field Artillery. 5. He was honorably released from active duty on 23 April 2010. He completed 1 year and 9 days of creditable active service during this period. 6. Item 13 of his DD Form 214 for the period ending 23 April 2010 shows the: * Army Good Conduct Medal * National Defense Service Medal * Global War on Terrorism Service Medal * Iraqi Campaign Medal with campaign star * Army Service Ribbon * Overseas Service Ribbon * Armed Forces Reserve Medal with "M" Device 7. Item 14 (Military Education) of his DD Form 214 for the period ending 23 April 2010 shows the entry "None," indicating no training was completed during the period covered by this DD Form 214. 8. He provides: a. Permanent Orders 303-004, issued by Headquarters, 1st Theater Sustainment Command, Camp Arifjan, Kuwait, dated 30 October 2009, awarding him the Combat Action Badge for actively engaging or being engaged by the enemy on 21 August 2009. b. Permanent Orders Number 104-01, issued by the Department of Military Affairs, COARNG, dated 14 April 2009, awarding him the Army Achievement Medal for achievement from 23 January to 16 March 2009. c. Permanent Orders 010-061, issued by Headquarters, 115th Fires Brigade, dated 12 January 2010, awarding him the Army Commendation Medal for meritorious service from 28 June 2009 through 15 April 2010. d. A certificate of training that show he completed the 3-day Counter Improvised Explosive Device Training Level III Course in Iraq from 26 to 28 October 2009. e. A certificate of training, dated 5 July 2009, which shows he completed the 40-hour MaxxPro Plus OPNET Course. 9. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. Chapter 2 of Army Regulation 635-5 contains guidance on the preparation of the DD Form 214. It states for: a. Item 13, list awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22. Each entry will be verified by the Soldier's records. b. Item 14, from the Soldier's Enlisted Record Brief, list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214. Include title, length in weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, training courses for combat skills are not listed. DISCUSSION AND CONCLUSIONS: 1. With respect to the awards, permanent orders awarded him the Army Commendation Medal, Army Achievement Medal, and the Combat Action Badge which are not shown on his DD Form 214 for the period ending 23 April 2010. Therefore, his DD Form 214 should be corrected to show these awards. 2. With respect to his training, the regulation requires the listing of formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 and includes title, length in weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, training courses for combat skills are not listed. It appears the requested courses are training courses for combat skills. Such training is not entered on the DD Form 214. Therefore, there is no basis for granting this portion of the applicant's request. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by adding to item 13 of his DD Form 214 for the period ending 23 April 2010 the: * Army Commendation Medal * Army Achievement Medal * Combat Action Badge 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
