THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Commendation Medal and two awards of the Army Achievement Medal. 2. The applicant states, in effect, he has two awards of the Army Achievement Medal and one of the awards is not shown on his DD Form 214. 3. The applicant provides two orders for award of the Army Achievement Medal and orders for the award of the Army Commendation Medal (1st Oak Leaf Cluster). CONSIDERATION OF EVIDENCE: 1. A review of item 13 (Decorations, Medals, Badges, Commendations, Citations and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 reveals it shows the Army Commendation Medal (2nd Award). As both awards of the Army Commendation Medal are shown on his DD Form 214, there is no correction to his records needed to add this award and it will not be discussed further in this record of proceedings. 2. The applicant's records show he enlisted in the Regular Army on 15 April 2003 and he held military occupational specialty 63H (Tracked Vehicle Mechanic). 3. On 21 September 2009, he was honorably released from active duty in the rank/grade of specialist/E-4 by reason of expiration of term of service and he was transferred to the U.S. Army Reserve Control Group to complete his remaining Reserve obligation. 4. Item 13 of his DD Form 214 shows the Army Commendation Medal (2nd Award), Joint Service Achievement Medal, Army Achievement Medal, Army Good Conduct Medal (2nd Award), National Defense Service Medal, Global War on Terrorism Expeditionary Medal, Global War on Terrorism Service Medal, Iraq Campaign Medal with one bronze service star, Army Service Ribbon, Overseas Service Ribbon with numeral 2, and the Driver and Mechanic Badge with Driver Tracked Vehicle Clasp. 5. The applicant provides two certificates, issued by Headquarters, 1st Battalion, 21st Field Artillery, awarding him two awards of the Army Achievement Medal as follows: * Permanent Orders Number 213-07 (undated), awarding him the Army Achievement Medal for meritorious service from 9 July to 18 July 2007 * Permanent Orders Number 319-02, dated 15 November 2007, awarding him the Army Achievement Medal for meritorious service from 16 August to 16 September 2007 DISCUSSION AND CONCLUSIONS: Permanent Orders awarded the applicant two awards of the Army Achievement Medal and his DD Form 214 shows only one award of the Army Achievement Medal. Therefore, he is entitled to correction of his records to show both of these awards.
