THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of item 14 (Military Education) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he completed the following schools: * Junior Officer Professional Development (JOPD) * Basic Officer Leaders Course (BOLC) A * Helicopter Overwater Survival Training (HOST) * Survival, Evasion, Resistance, and Escape (SERE) Level B * SERE Level C * Initial Entry Rotary Wing (IERW) Course 2. The applicant states his DD Form 214 does not show the schools he attended as a warrant officer. 3. The applicant provides: * a Certificate of Completion for HOST, dated 20 May 2008 * a Certificate of Completion for SERE Level B, dated 11 May 2008 * a Certificate of Completion for SERE Level C, dated 11 May 2008 CONSIDERATION OF EVIDENCE: 1. With prior enlisted service in the Regular Army (RA) and the U.S. Army Reserve (USAR), the applicant was ordered to active duty effective 30 January 2008 for the purpose of attending Warrant Officer Candidate School (WOCS). 2. He was honorably discharged on 17 March 2008, to accept an appointment as a warrant officer in the USAR. Item 14 of the DD Form 214 for this period shows the entry “WOCS, 6 Weeks, 2008.” 3. The applicant accepted an appointment in the USAR, with concurrent active duty, in the rank of warrant officer one (WO1), effective 18 March 2008, for an indefinite term. 4. His records contain a memorandum from the commanding general of Headquarters, U.S. Army Aviation Center of Excellence, Fort Rucker, AL, subject: Involuntary Release from Active Duty (REFRAD) - (applicant's name), dated 1 July 2009. The memorandum states that on 28 May 2009 the applicant was eliminated from the IERW Course due to personal misconduct between 3 June and 6 December 2008. The memorandum further directed his REFRAD and termination of his warrant officer appointment. 5. He was REFRAD on 24 July 2009 by reason of failure to complete course of instruction. Item 14 of the DD Form 214 for this period shows the entry “None.” 6. The Certificates of Completion the applicant submits show he completed a 1-day HOST Certification Course on 20 May 2008. He completed a 5-hour SERE Level B Course on 11 May 2008 and a 363-hour SERE Level C Course on 11 May 2008. 7. Army Regulation 635-5 (Separation Documents) prescribes policies and procedures regarding separation documents. Paragraph 2-4 (Completing the DD Form 214) provides detailed instructions and source document(s) for completing each block of the DD Form 214. It provides that in item 14 list formal in-service (full-time attendance) training courses of 40 hours or more successfully completed during the period of service covered by the DD Form 214 from the Officer Record Brief (ORB). Include title, length in weeks, and year completed. This information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills. When in doubt, refer to the course description in Department of the Army Pamphlet (DA Pam) 351-4 (U.S. Army Formal Schools Catalog) to determine its usefulness to the Soldier after transitioning from the Army. DISCUSSION AND CONCLUSIONS: 1. The evidence of record shows he was eliminated from the IERW Course on 28 May 2009 due to personal misconduct. He did not successfully complete this course; therefore, there is no basis for adding this course on his DD Form 214. 2. The Certificate of Completion for HOST is less than the required 40 hours of training for inclusion on the DD Form 214. Therefore, there is no basis for adding this course on his DD Form 214. 3. The SERE Level B and SERE Level C courses are both combat-related courses. Therefore, there is no basis for adding these courses to his DD Form 214. 4. There is no evidence of record and the applicant did not provide any evidence that confirms he completed the JOPD and the BOLC-A. These courses are also not listed on his ORB. Therefore, there is no basis for granting this portion of the applicant's request.
