THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests removal of duplicate Army Achievement Medal (AAM) and Bronze Star Medal (BSM) Certificates from his official military personnel file in the interactive Personnel Electronic Records Management System (iPERMS). 2. The applicant states better-quality copies of an AAM Certificate and a BSM Certificate dated 30 April 2003 were placed in his iPERMS record. 3. The applicant provides no documentation with his request. CONSIDERATION OF EVIDENCE: 1. The applicant is a Regular Army Sergeant Major. 2. His record in iPERMS includes two identical certificates showing he was awarded the AAM for the period 26 October 2002 to 19 March 2003. iPERMS shows a different effective date for each certificate. The record copy of the certificate with the earlier effective date of 9 March 2003 is of poor quality. 3. His record in iPERMS includes two identical certificates showing he was awarded the BSM with V Device for the period 20 March to 30 April 2003. iPERMS shows a different effective date for each certificate. The record copy of the certificate with the earlier effective date of 30 April 2003 is of poor quality. 4. Army Regulation 600-8-104 (Military Personnel Information Management Records) prescribes the policies and mandated operating tasks for the Military Personnel Information Management/Records Program of the Military Personnel System. It provides principles of support, standards of service, policies, tasks, rules, and steps governing all work required in the field to support MILPER Information Management/Records. It provides that once placed in the OMPF, the document becomes a permanent part of that file. The document will not be removed from a fiche or moved to another part of the fiche unless directed by the ABCMR. DISCUSSION AND CONCLUSIONS: 1. The applicant's record contains duplicate copies of the AAM and BSM Certificates. This duplication has the potential to be a source of confusion. 2. As a matter of equity, he is entitled to have the poor-quality copies with the effective dates of 9 March 2003 and 20 April 2003 removed from his record.
