THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show in block 12a that he entered active duty on 28 May 1991. 2. The applicant states that his DD Form 214 incorrectly shows he entered active duty on 3 July 1991 instead of 28 May 1991 and represents a break in service that never occurred. 3. The applicant provides copies of: * his DD Forms 214 * DA Form 7301-R (Officer Service Computation for Retirement) * orders awarding him aviation pay on 28 May 1991 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 7 July 1983 for a period of 4 years. He continued to serve through continuous reenlistments as a special forces communications sergeant and he was promoted to pay grade E-6 on 1 June 1986. On 27 May 1991, he was honorably discharged to accept a warrant officer appointment. 2. On 28 May 1991, he was appointed as a warrant officer one (WO1) with a concurrent call to active duty. On 30 September 2009, he was retired in the rank of chief warrant officer four (CW4). He had completed 18 years, 4 months, and 3 days of active service this period and 26 years, 2 months, and 24 days of total active service. 3. His DD Form 214 issued at the time of his retirement reflects in block 12a that he entered active duty this period on 3 July 1991; however, his service computation in block 12c. (0018 04 03) is computed from 28 May 1991. 4. The applicant’s DA Form 7301-R also reflects he began his active duty warrant officer service on 28 May 1991. DISCUSSION AND CONCLUSIONS: 1. The applicant’s contention that his DD Form 214 incorrectly reflects that he entered active duty this period on 3 July 1991 has been noted and found to have merit. 2. The applicant was appointed as a WO1 on 28 May 1991 with a concurrent call to active duty and he served on active duty until he retired on 30 September 2009. 3.
