THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to reflect his deployments to Southwest Asia and Iraq. 

2.  The applicant states that he deployed to Southwest Asia (SWA) from            23 December 1990 to 18 May 1991 and to Iraq in support of Operation Iraqi Freedom from 21 November 2005 to 18 November 2006 and his deployments were not added to his DD Form 214 at the time of his retirement.

3.  The applicant provides:
* A copy of his DD Form 214
* A copy of his DA Form 2-1 (Personnel Qualification record – Part II)
* A copy of his deployment orders
* A copy of his Enlisted Records Brief (ERB)  

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 24 February 1983 and served through a series of continuous reenlistments.  He was promoted to the pay grade of E-8 on 1 December 2000.

2.  His records show that he deployed to SWA from 19901201 - 19910519 and to Iraq from 20051122 – 20061116. 

3.  On 31 December 2008 he was honorably released from active duty (REFRAD) and was transferred to the Retired List effective 1 January 2009.  He had served 25 years, 10 months and 7 days of total active service.   

4.  His DD Form 214 issued at the time of his REFRAD does not reflect his deployments in the remarks section (block 18).

5.   Army Regulation 635-5 serves as the authority for the preparation of the DD Form 214.  It provides, in pertinent part, that it is mandatory that an entry be placed in the remarks section of the DD Form 214 for an active duty Soldier deployed with his or her unit during their continuous period of active service.  The entry will consist of “Service in (Name of Country deployed from YYYYMMDD – YYYYMMDD)”.

DISCUSSION AND CONCLUSIONS:

1.  The evidence of record shows that the applicant deployed to SWA and Iraq in support of Operation Iraqi Freedom.  Accordingly, he is entitled to have those deployments reflected on his DD Form 214. 

2.  Accordingly, the entry “Service in SWA  19901201 - 19910519 and Service in Iraq 20051122 – 20061116” should be entered on his DD Form 214 at this time.

BOARD DETERMINATION/RECOMMENDATION:

1. The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing in the remarks section of his DD Form 214 (block 18) the entry “Service in SWA  19901201 - 19910519 and Service in Iraq 20051122 – 20061116”.


2.  The Board wants the applicant and all others to know that the sacrifices he made in service to the United States during the Global War on Terrorism are deeply appreciated.  The applicant and all Americans should be justifiably proud of his service in arms.
