THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the Bronze Star Medal With "V" (valor) Device and to show the word "Device" instead of the word "Devise." He also requests correction a Servicemembers' Group Life Insurance (SGLI) Election and Certificate to show the correct spelling of his sister's first name. 2. The applicant states the award was received after discharged from active duty and the word device is misspelled on his DD Form 214. He adds that his sister's name is misspelled on the SGLI Election and Certificate. 3. The applicant provides a DD Form 214, a Bronze Star Medal with "V" Device Certificate, and an SGLI Election and Certificate. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 12 September 2002. He completed his initial entry training and was awarded the military occupational specialty 11B (Infantryman). 2. He was released from active duty and transferred to the Army National Guard (ARNG) on 22 November 2008. Item 13 of his DD Form 214 shows he was awarded the Joint Service Commendation Medal with "V" Device; however, the spelling of the word device is shown as "devise." 3. Permanent Orders Number 366-001, dated 31 December 2008, awarded him the Bronze Star Medal with "V" Device for exceptionally valorous achievement on 9 June 2008. 4. He served in the Army National Guard from 23 November 2008 to 31 July 2010. He no longer holds a military status. 5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. a. It establishes standardized policy for the preparation of the DD Form 214. b. It states the DD Form 214 is a summary of the Soldier's most recent period of continuous active service. It provides a brief, clear-cut record of active service at the time of release from active duty, retirement, or discharge. c. It provides that for item 13, list awards and decorations for all periods of service in the priority sequence specified in Army Regulation 600-8-22 (Military Awards). Each entry will be verified by the Soldier’s records. 6. The SGLI Election and Certificate is for use by Active Duty and Reserve Members. This form does not apply to and cannot be used for any other Government Life Insurance. DISCUSSION AND CONCLUSIONS: 1. He was awarded the Bronze Star Medal with 'V" Device after his separation from active and as a result is not shown on his DD Form 214. Therefore, it would be appropriate to correct item 13 of his DD Form 214 to show this award. 2. He was awarded the Joint Service Commendation Medal with "V" Device; however, an error was made in the preparation of his DD Form 214 in that the word "device" was spelled as "devise." It would be appropriate to correct the spelling of this word. 3. He also request correction of the SGLI Election and Certificate to show the correct spelling of his sister's first name; however, this form is for use by Active Duty and Reserve Members. Because he no longer holds a military status, the form is no longer of any value. Therefore, this portion of his request should be denied. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. deleting from item 13 of his DD Form 214 the Joint Service Commendation Medal with "V" Devise; and b. adding to item 13 of his DD Form 214 the Joint Service Commendation Medal with "V" Device and the Bronze Star Medal with "V" Device. 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
