THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of item 12a (Date Entered Active Duty (AD) This Period) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he entered AD on 18 March 2004. 2. The applicant states: * He has paperwork showing item 12a is incorrect * He went through “basic” in 2004 * He had 3 1/2 years of AD service 3. The applicant provides: * Military Entrance Processing Station Orders 40782, dated 18 March 2002 * Headquarters, U.S. Army Infantry Center Order 153-3166, dated 1 June 2004 CONSIDERATION OF EVIDENCE: 1. The applicant’s DD Form 4/3 (Enlistment/ Reenlistment Document Armed Forces of the United States) shows he enlisted in the Regular Army for 3 years and 16 weeks on 18 March 2004. He completed training as an infantryman. 2. On 9 September 2007, the applicant was honorably released from active duty (REFRAD). He had completed 3 years, 5 months, and 22 days of net active service this period. 3. Item 12a on his DD Form 214 shows he entered AD this period on 18 March 2005. 4. The applicant submits Military Entrance Processing Station Orders 40782, assigning him to the 30th Adjutant General Battalion (Reception), Fort Benning, Georgia, effective 19 March 2004. Headquarters, United State Army Infantry Center Orders 153-3166 show he had a permanent change of station to the 10th Mountain Division Replacement Detachment, effective 5 August 2004. 5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents which are prepared for individuals upon retirement, REFRAD, or discharge. It establishes standardized policy for preparing and distributing the DD Form 214. The DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. It provides a clear-cut record of active duty service at the time of release from active duty, retirement, or discharge. It states in item 12a, enter the beginning date of the continuous period of active duty for issuance of this DD Form 214. DISCUSSION AND CONCLUSIONS: 1. The applicant’s contentions have been considered. 2. It is evident that an error was made during the preparation of his DD Form 214 which resulted in the incorrect date being entered in Item 12a. 3. His DA Form 4/3 shows he enlisted in the RA on 18 March 2004. His DD Form 214 erroneously shows he entered AD on 18 March 2005. Item 12a on his DD Form 214 should be amended to show he entered AD this period on 18 March 2004.
