Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge), dated 1 January 2010, be corrected to reflect the following awards: * One award of the Army Achievement Medal (AAM) * One award of the Armed Forces Expeditionary Medal (AFEM) * Two awards of the Armed Forces Service Medal (AFSM) * Removing the Navy Arctic Service Ribbon 2. The applicant states his DD Form 214 incorrectly reflects he was awarded two awards of the AAM and AFEM. It also incorrectly shows he was awarded the Navy Arctic Service Ribbon. However, it does not show he was awarded the AFSM when in fact he has two awards of the AFSM. 3. The applicant provides copies of his DD Forms 214 issued on 27 February 1999 and 1 January 2010. CONSIDERATION OF EVIDENCE: 1. The applicant served in the United States Marine Corps from 28 March 1991 to 27 March 1995 when he was honorably released from active duty (REFRAD) due to completion of required service. His DD Form 214 issued at the time of his REFRAD shows that he was awarded the National Defense Service Medal (NDSM), the Good Conduct Medal, the AFEM, and the Sea Service Deployment Ribbon with Numeral 1. 2. On 28 February 1996, he enlisted in the Regular Army in Heidelberg, Germany for a period of 3 years for training as an infantryman and assignment to Europe. He served on active duty until he was honorably discharged on 27 February 1999 due to completion of required active service. His DD Form 214 issued at the time of his discharge shows that he was awarded the AAM, the Good Conduct Medal (2d Award), the NDSM, the Army Service Ribbon, the Overseas Service Ribbon, the Sea Service Deployment Ribbon with Numeral 1, the German Marksmanship Badge in Gold, the NATO Medal, the AFSM w/1OLC, and the AFEM. 3. He enlisted in the United States Army Reserve (USAR) in the pay grade of E-4 on 18 April 2007 for a period of 6 years. He was promoted to the pay grade of E-5 on 1 September 2007 and to the pay grade of E-6 on 1 September 2008. 4. On 2 January 2009, he was ordered to active duty in support of Operations Iraqi/Enduring Freedom. He deployed to Iraq from 27 August 2009 to 25 November 2009. 5. On 1 January 2010 he was honorably REFRAD due to completion of required active service. He had served 1 years of active service this period and his DD Form 214 issued at the time of his REFRAD reflects that he was awarded the Army Commendation Medal (2nd award), the Joint Service Achievement Medal, the AAM (2nd award), Army Good Conduct Medal (2nd award), Marine Corps Good Conduct Medal, the NDSM (2nd award), AFEM (2nd award), the Global War on Terrorism Service Medal, the Iraq Campaign Medal with campaign star, the Army Service Ribbon (2nd award), the Overseas Service Ribbon (2nd award), the Navy Arctic Service Ribbon, the Armed Forces Reserve Medal with “M” Device, the Navy Sea Service Deployment Ribbon (2nd award), the NATO Medal (2nd award) and the German Armed Forces Achievement Badge Gold. 6. A review of the available records failed to show that the applicant received two awards of the AAM the AFEM and the Army Service Ribbon or that he was awarded the Navy Arctic Service Ribbon. His records do show that he was awarded two awards of the Armed Forces Service Medal that was omitted from his DD Form 214. DISCUSSION AND CONCLUSIONS: 1. The applicant’s contentions have been noted and found to have merit. 2. His DD Form 214 issued on 1 January 2010 incorrectly reflects that he was awarded the AAM (2nd award), the AFEM (2nd award), the Army Service Ribbon (2nd award), and the Navy Arctic Service Ribbon. 3. Accordingly, his records should be corrected to show that he was awarded the AAM, the AFEM, and the Army Service Ribbon, by deleting the award of the Navy Arctic Service Ribbon, and adding the award of the Armed Forces Service Medal (2nd award).
