THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that item 12a (Date Entered Active Duty This Period) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) for the period ending 14 December 2009 be changed from 26 August 2007 to 15 December 2008. 2. The applicant states this information is incorrect because she entered active duty at Letterkenny Army Depot on 15 December 2008 and served until 14 December 2009 under Title 10 active duty for operational support (ADOS) orders. 3. The applicant provides her DD Form 214 for the period ending 14 December 2009; email transmissions; assignment orders, dated 17 December 2008; and separation orders, dated 7 December 2009. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the U.S. Army Reserve (USAR) on 17 February 1995. At the time she submitted her application, she was assigned to a USAR troop program unit (TPU) in the rank of sergeant. 2. U.S. Army Human Resources Command, St. Louis, MO (HRC-STL), published orders on 17 December 2008 which ordered the applicant to active duty with a reporting date of 15 December 2008 for the purpose of ADOS. 3. Headquarters, Carlisle Barracks, Carlisle, PA, published orders on 7 December 2009 which released the applicant from active duty to the control of her TPU in Charleston, SC, effective 14 December 2009. 4. Item 12a of her DD Form 214 for the period ending 14 December 2009 shows she entered active duty on 26 August 2007 and was released from active duty on 14 December 2009, which equals 2 years, 3 months, and 19 days of net active service during this period. However, item 12c (Net Active Service This Period) of this form shows she completed 1 year and 1 day of net active service during this period. 5. In a 14 April 2010 email, a staff member from the HRC-STL Force Sustainment Branch informed the applicant that her retirement points statement had been corrected and updated. She was also informed that item 12a of her DD Form 214 needed to be corrected to read 15 December 2008. She was advised to contact to the mobilization station or transition point in order to have this correction made. 6. In a 16 April 2010 email, the Carlisle Barracks Retirement Services Officer informed the applicant that her DD Form 214 had been corrected in the transition processing database. She was advised to submit an application to this Board for correction of her DD Form 214. 7. Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214. It states that the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge. In pertinent part, it directs that the beginning date of the continuous period of active duty will be entered in item 12a for issuance of this DD Form 214 for which a DD Form 214 was not previously issued. It further directs that for item 12c, enter the amount of service this period, computed by subtracting item 12a from item 12b (Separation Date This Period), less lost time, if any. DISCUSSION AND CONCLUSIONS: 1. Orders published by HRC-STL ordered the applicant to active duty effective 15 December 2008 and she was released from active duty on 14 December 2009. However, her DD Form 214 for the period ending 14 December 2009 erroneously reflects she entered active duty on 26 August 2007. 2. Therefore, it would be appropriate to amend item 12a of the applicant's DD Form 214 for the period ending 14 December 2009 to show she entered active duty on 15 December 2008.
