THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that the Unit Prevention Leader Certificate, dated 28 May 2008, be removed from his Official Military Personnel File (OMPF). 2. The applicant states, in effect, that the certificate with the 28 May 2008 date is in error. The correct copy is dated 28 May 2004 and it has been posted to his record. He states he requested that the certificate be reissued because the original copy was stained and was unable to be submitted for filing in his record. When the certificate was reissued it contained an error and he did not see the error until the certificate had been posted in his record. 3. The applicant provides two copies of Unit Prevention Leader Certificates of Training with ending dates of 28 May 2008 and 28 May 2004 in support of his application. CONSIDERATION OF EVIDENCE: 1. The applicant is currently serving on active duty in the Regular Army (RA) in the rank of sergeant first class, E-7. 2. Headquarters, I Corps and Fort Lewis, WA issued the applicant a Certificate of Training which shows he completed the Unit Prevention Leader Course from 24 May 2004 to 28 May 2008 (40 hours). 3. Headquarters, I Corps and Fort Lewis issued the applicant a second Certificate of Training which shows he completed the Unit Prevention Leader Course from 24 May 2004 to 28 May 2004 (40 hours). 4. Army Regulation 600-8-104 (Military Personnel Information Management/ Records) prescribes the policies governing the OMPF, the Military Personnel Records Jacket, the Career Management Individual File, and Army Personnel Qualification Records. Paragraph 2-4 of this regulation states that once a document is placed in the OMPF it becomes a permanent part of that file and will not be removed from that file or moved to another part of the file unless directed by the Army Board for Correction of Military Records, the Department of the Army Suitability Evaluation Board (DASEB), Army Appeals Board, Chief of Appeals and Corrections Branch of the U.S. Army Human Resources Command, the OMPF custodian when documents have been improperly filed, Human Resources Command, as an exception the Chief of the Appeals Branch of the Army Reserve Personnel Center, and the Chief of the Appeals Branch of the National Guard Personnel Center. DISCUSSION AND CONCLUSIONS: 1. The applicant contends that the certificate with an ending date of 28 May 2008 is in error and that it should be removed from his record. He contends the correct certificate has an ending date of 28 May 2004 and it has already been posted on his record. 2. Headquarters, I Corps and Fort Lewis issued the applicant a Certificate of Training that shows he completed the Unit Prevention Leader Course during the period 24 May 2004 to 28 May 2008 (40 hours) which is clearly in error. 3. Headquarters, I Corps and Fort Lewis issued the applicant a second Certificate of Training which properly shows he completed the Unit Prevention Leader Course during the period 24 May 2004 to 28 May 2004 (40 hours). 4. The Army has an interest in maintaining the accuracy of its records. It is also important that these records be as concise as possible to help preclude misinterpretation by individuals or boards during the review of these records. Therefore, it would be appropriate to remove the Unit Prevention Leader Course Certificate for the period 24 May 2004 to 28 May 2008 (40 hours) from the applicant's OMPF.
