THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of Items 12c (Net Active Service This Period), 12d (Total Prior Active Service), and 12e (Total Prior Inactive Service) on her 23 April 1999 DD Form 214 (Certificate of Release or Discharge from Active Duty). 2. The applicant states that her net active service, total prior active service, and total prior inactive service are not updated on her DD Form 214. 3. The applicant provides a copy of her DD Form 214 for the period ending 23 April 1999. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the U.S. Army Reserve on 27 August 1998 for a period of eight years. 2. The applicant was ordered to active duty for training (ADT) on 12 November 1998. 3. The applicant was released from ADT on 23 April 1999 at the completion of her required active service. 4. Item 12a of her DD Form 214 shows she entered active duty on 12 November 1998. 5. Item 12c of her DD Form 214 shows she completed 5 months and 12 days net active service during the period covered by the report. 6. Item 12d of her DD Form 214 does not reflect any prior active service. 7. Item 12e of her DD Form 214 shows 2 months and 15 days prior inactive service. 8. Army Regulation 635-5 (Separation Documents) governs the preparation of the DD Form 214. It states that the DD Form 214 is a synopsis of the Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active Army service at the time of release from active duty, retirement or discharge. In pertinent part it directs that total prior active service less lost time will be entered in item 12d. It also directs that the total prior inactive service less lost time will be taken from previously issued DD Forms 214 and the total amount will be entered in item 12e. DISCUSSION AND CONCLUSIONS: 1. The applicant contends that her net active service, total prior active service, and total prior inactive service are not updated on her DD Form 214. However, there is insufficient evidence to substantiate her claim. 2. The applicant enlisted in the U.S. Army Reserve on 27 August 1998 and completed 5 months and 12 days of ADT from 12 November 1998 to 23 April 1999. At the completion of ADT, she was issued a DD Form 214 which properly reflects 5 months and 12 days active military service in Item 12c. Therefore, there is no basis for amending Item 12c of her DD Form 214. 3. There is no evidence of record which shows the applicant completed any active service prior to the date she was ordered to ADT on 12 November 1998. Therefore, there is insufficient evidence on which to amend Item 12d of her DD Form 214 to reflect prior active service. 4. The evidence of record shows the applicant completed 2 months and 15 days prior inactive service, which covers the date she enlisted in the U.S. Army Reserve on 27 August 1998 to 11 November 1998. There is no evidence of record which shows she served any additional prior inactive service. Therefore, there is no basis for amending Item 12e of her DD Form 214.
