THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that Item 18 (Remarks) of her DD Form 214 Certificate of Release or Discharge from Active Duty) be amended to show she served her tour of duty in Iraq. 2. The applicant states, in effect, that Item 18 shows she served her tour of duty in Kuwait when she actually served in Mosul, Iraq, for her tour. She also states, in effect, during her final interview she gave Kuwait as the country since this was where her company started out from before going to various forward operating bases in Northern Iraq. At the end of their tour they flew to Kuwait and stayed a few days before returning to the United States. 3. In support of her application, the applicant provides copies of her officer evaluation report, her Army Commendation Medal certificate, her DD Form 214, and her DD Form 2796 (Post-Deployment Health Assessment). CONSIDERATION OF EVIDENCE: 1. The applicant's military records show she entered on active duty from the United States Army Reserve on 31 December 2008 in support of Operation Iraqi Freedom. 2. The applicant submits a copy of her officer evaluation report for the period from 24 October 2008 to 10 April 2009 that shows she was serving with the 360th Medical Company (Dental Support), in Iraq, as a general dental officer. She also submits a copy of her Army Commendation Medal certificate showing she received this award for her exceptionally meritorious service while serving in Iraq. 3. The applicant was honorably released from active duty on 4 May 2009 and transferred to a Reserve unit. Item 18 of her DD Form 214 shows she served in Kuwait from 10 January 2009 to 19 April 2009. It does not show she served in Iraq. 4. A staff member of the Defense Finance and Accounting Service, Indianapolis, Indiana, verified that the applicant served in Iraq and received hostile fire pay from 1 January 2009 to 30 April 2009 (pay is calculated from the first day of the month the tour begins to the last day of the month the tour ends). 5. Army Regulation 635-5 (Separation Documents) specifies that a DD Form 214 would be prepared for individuals who were retired, discharged, or released from released from active duty. Item 18 will list the country and dates of deployment. DISCUSSION AND CONCLUSIONS: 1. The evidence of records shows the applicant was deployed to Iraq from 10 January 2009 to 19 April 2009. Therefore, it is concluded that Item 18 of her DD Form 214 should be corrected to show the entry, "Service in Iraq from 20090110 to 20090419." 2. In view of the foregoing, the applicant’s records should be corrected as recommended below.
