THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that a DA Form 1059 (Service School Academic Evaluation Report), dated 24 July 2008, that shows he achieved course standards be removed from his Official Military Personnel File (OMPF). 2. The applicant states that there are two DD Forms 1059, dated 24 July 2008, in his OMPF. One shows he completed Drill Sergeant School (DSS) and the other shows he did not complete the course. He states that the DA Form 1059 that shows he did not complete DSS is correct and the DA Form 1059 that shows he completed the course needs to be removed from his OMPF. 3. The applicant states that he was medically released from the course. He states that he pre-signed the two DA Forms 1059 and that the school made a mistake by submitting both documents to be added in his OMPF. 4. The applicant provides two DA Forms 1059 with the date 24 July 2008. CONSIDERATION OF EVIDENCE: 1. The performance portion of the applicant's OMPF has two DA Forms 1059 dated 27 July 2008, one shows he "ACHIEVED COURSE STANDARDS" and one shows that he "FAILED TO ACHIEVE COURSE STANDARDS" for Drill Sergeant Course 12-08. 2. On 4 August 2009, the operations noncommissioned officer (NCO) at the U.S. Army DSS at Fort Jackson, South Carolina verified to the Board analyst that the applicant was dropped from the DSS course on graduation day, due to being issued a physical profile for a stress fracture. DISCUSSION AND CONCLUSIONS: Evidence of record shows that a DA Form 1059, dated 24 July 2008, showing the applicant had achieved course standards for Drill Sergeant Course 12-08 was incorrectly placed in the applicant's OMPF for no apparent reason. The school operations NCO verified that the applicant was dropped from the course and the DA Form 1059 was erroneously placed in the applicant's OMPF. Therefore, the DA Form 1059, dated 24 July 2008, that shows the applicant achieved course standards should be removed from the performance portion of his OMPF.
