THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his records to show his award of the Combat Infantryman Badge and Army Commendation Medal. 2. The applicant states the Combat Infantryman Badge and Army Commendation Medal are missing from his service record. 3. The applicant provides copies of Headquarters, Multi-National Division (Baghdad), Camp Liberty, Iraq, Permanent Orders Number 135-028, dated 15 May 2006; an Army Commendation Medal certificate for the period from 29 November 2005 to 15 November 2006; and a copy of his DD Form 214 for the period ending 30 July 2008. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army (RA) on 15 November 2004, he completed training, and he was awarded military occupational specialty (MOS) 11B (Infantryman). 2. The applicant was honorably discharged under Army Regulation 635-200 (Personnel Separations – Active Duty Enlisted Administrative Separation), paragraph 5-17 for a physical or mental condition not amounting to a disability. He had 3 years, 8 months, and 16 days of creditable service. 3. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of his DD Form 214 shows his authorized awards as the National Defense Service Medal, Global War on Terrorism Service Medal, Iraq Campaign Medal with one campaign [bronze service] star, Army Service Ribbon, and the Overseas Service Ribbon. 4. Permanent Orders Number 135-028 awarded the applicant the Combat Infantryman Badge for his service in ground combat in Iraq in support of Operation Iraqi Freedom on 18 February 2006. 5. The Army Commendation Medal certificate shows the applicant was awarded the Army Commendation Medal for meritorious service while deployed in support of Operation Iraqi Freedom from 29 November 2005 through 15 November 2006 in accordance with Permanent Order Number 155-064. DISCUSSION AND CONCLUSIONS: 1. The applicant states the Combat Infantryman Badge and Army Commendation Medal are missing from his service record. 2. The applicant's official record contains a copy of the orders awarding him the Combat Infantryman Badge. Therefore, his records should be corrected to show this award. 3. The applicant's record does not contain a copy of the Army Commendation Medal recommendation or certificate. However, based on his dates of service in Iraq and the inclusion of the Permanent Orders Number on the award certificate he provided, it is reasonable to assume that this is a valid certificate. Therefore, it would be appropriate to correct the record to show this award.
