THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show that he completed the Survive, Evade, Resist, and Escape (SERE) High Risk (Level C) Course at the U.S. Army John F. Kennedy Special Warfare Center and School. He also requests that the Shoulder Sleeve Insignia-Former Wartime Service (SSI-FWTS) be added to his DD Form 214. 2. The applicant states that the service school and campaign medal were not entered into the system or on his DD Form 214. 3. The applicant provides a Diploma from the U.S. Army John F. Kennedy Special Warfare Center and School, dated 5 May 2000, shows he graduated from the SERE High Risk (Level C) Course. He also provided a memorandum, dated 24 August 2006, in support of his application. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 7 October 1994 and continued to serve on active duty through two reenlistments. 2. A memorandum, dated 24 August 2006, shows applicant was authorized to wear the SSI-FWTS for duty in Bagdad, Iraq while assigned to the CJSOTF-AP, 7th Special Forces Group (Airborne). 3. The applicant was discharged on 27 July 2006 at the completion of his required period of active service. At the time of his discharge, he had completed 11 years, 9 months, and 21 days of active military service. 4. Army Regulation 635-5 prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. The instructions for completion of item 14 in the regulation in effect at the time were to list in-service training courses (title, number of weeks, and year successfully completed during the period of service covered by the DD Form 214. This information was to assist the member after separation in job placement and counseling; therefore, training courses for combat skills would not be listed. 5. Army Regulation 670-1 (Wear and Appearance of Army Uniforms and Insignia) prescribes the authorization for wear, composition, and classification of uniforms, and occasions for wear for all personal (clothing bag issue), optional and commonly worn organizational Army uniforms. This regulation also prescribes the awards, insignia, and accouterments authorized for wear on the uniform, and how these items will be worn. 6. Paragraph 27-17 of Army Regulation 670-1 provides authorization to wear a shoulder sleeve insignia indicating former wartime service. DISCUSSION AND CONCLUSIONS: 1. Based on Army Regulation 635-5, training courses for combat skills are not listed on the DD Form 214. Therefore, the applicant’s SERE High Risk (Level C) Course is not authorized to be added to item 14 of his DD Form 214. 2. Based on Army Regulation 670-1, the SSI-FWTS is an item to be worn on the Soldier’s uniform and is not an award or decoration. Since the SSI-FWTS is not considered an award or decoration, it is not authorized to be entered on the DD Form 214. However, the applicant is authorized to wear this item.
