THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show a second deployment. 2. The applicant states that his second deployment is not recorded on his DD Form 214 and that it may have been overlooked. 3. The applicant provides a copy of his DD Form 214 and a Soldier Deployment History Outprocessing Report in support of his application. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 2 January 2003 for a period of 6 years. He trained as an armor crewman and was honorably released from active duty in the rank of sergeant on 1 January 2009. 2. Item 18 (Remarks) on the applicant's DD Form 214 shows that he served in Iraq from 21 January 2005 to 24 December 2005. 3. In support of his claim, the applicant provided a Soldier Deployment History Outprocessing Report which shows he was deployed in January 2005 to January 2006 and from 7 January 2007 to 31 January 2008. 4. Records at the Defense Finance and Accounting Service (DFAS) show the applicant served in a combat zone receiving hostile fire pay during the period 1 January 2007 to 31 March 2008 in Kuwait and his inclusive dates of service were 7 January 2007 to 26 March 2008. 5. The applicant's Noncommissioned Officer Evaluation Report (NCOER) covering the period 1 December 2007 to 30 November 2008 states, in pertinent part, "combat in Iraq." 6. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be entered in item 18. DISCUSSION AND CONCLUSIONS: The DFAS information, combined with the applicant's Soldier Deployment History Outprocessing Report which states he served in Kuwait/Iraq from 7 January 2007 to 26 March 2008, is accepted as sufficient evidence on which to amend item 18 on the applicant’s DD Form 214 to show his second deployment in Kuwait/Iraq. Therefore, item 18 on his DD Form 214 should be corrected to show he served in Kuwait/Iraq from 7 January 2007 to 26 March 2008.
