THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, that the Meritorious Service Medal certificate in his Official Military Personnel File (OMPF) with an incorrect name in the citation narrative be removed. 2. The applicant essentially states that the Meritorious Service Medal awarded to him by 20th Support Command Permanent Orders Number 116-7, dated 26 April 2008, contains another Soldier's name in the citation narrative. He also states that the 20th Support Command issued him a new Meritorious Service Medal in Permanent Orders Number 116-1, dated 26 April 2008, and added it to his OMPF, but they did not revoke or remove the incorrect award certificate from his OMPF. 3. The applicant provides both of the aforementioned Meritorious Service Medal certificates in support of this application. CONSIDERATION OF EVIDENCE: 1. The applicant is currently a sergeant first class in the Regular Army. 2. A review of the applicant’s OMPF shows that there are two certificates for award of the Meritorious Service Medal for the period 7 June 2001 to 6 June 2008. The certificate with 20th Support Command Permanent Orders Number 116-7 incorrectly contains the last name of another Soldier in the citation narrative. DISCUSSION AND CONCLUSIONS: Evidence of record clearly shows the Meritorious Service Medal certificate in the applicant's OMPF with 20th Support Command Permanent Orders Number 116-7 incorrectly contains the last name of another Soldier in the citation narrative, and that this certificate was reissued under Permanent Orders Number 116-1 with the applicant's name in the citation narrative. However, the incorrect award certificate was not removed from his OMPF at the time the correct award certificate was posted. Therefore, it would be appropriate at this time to remove the Meritorious Service Medal certificate that contains 20th Support Command Permanent Orders Number 116-7 from his OMPF.
