THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that item 12f (Foreign Service) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show his service in Iraq. He also requests that his DD Form 214 be corrected to show that he completed Airborne School. 2. The applicant states that he was deployed to Iraq from 5 August 2003 to 10 April 2004, that his DD Form 214 is inaccurate, and that the civilians in the out-processing center would not allow him a reasonable amount of time to retrieve the necessary documents. He also states that his DD Form 214 does not show he completed Airborne School. 3. The applicant provides two applications, a Soldier Deployment History Outprocessing Report, two DA Forms 638 (Recommendation for Award), an Enlisted Record Brief, a diploma, a DA Form 1307 (Individual Jump Record) and a copy of his DD Form 214 in support of his applications. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 13 February 2003. He served as an infantryman in Iraq during the period 5 August 2003 to 10 April 2004. On 10 January 2006, he was released from active duty. 2. Item 12f on the applicant's DD Form 214 does not show any foreign service. Item 14 (Military Educations) on his DD Form 214 shows the entry "NONE//NOTHING FOLLOWS." Item 18 (Remarks) on his DD Form 214 does not show any deployments. 3. The applicant provided a diploma, dated 20 June 2003, which shows he completed the Airborne Course at Fort Benning, Georgia. 4. The applicant provided a Soldier Deployment History Outprocessing Report which shows he was deployed from 5 August 2003 to 10 April 2004 (a total of 8 months and 6 days). He also provided a DA Form 638, dated 4 December 2005, for award of the Army Achievement Medal which states, in pertinent part, that he participated in both Operation Enduring Freedom and Operation Iraqi Freedom and that he led combat operations in Lutifiyah and Mahudiyah. 5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that, for Item 12f, from the Soldier's records, enter the total amount of foreign service completed during the period covered in Item 12c (Net Active Service This Period). The regulation provides that item 14 will list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by title, length in weeks, and year completed. Training courses for combat skills will not be listed. The regulation also states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be entered in item 18. DISCUSSION AND CONCLUSIONS: 1. The Soldier Deployment History Outprocessing Report and the DA Form 638 provided by the applicant which show he served in Iraq from 5 August 2003 to 10 April 2004 are accepted as sufficient evidence in which to amend items 12f and 18 of the applicant’s DD Form 214. 2. Evidence of record shows the applicant successfully completed the Airborne Course in June 2003. Therefore, it would be appropriate to correct item 14 on his DD Form 214 to show this course.
