THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show a third award of the Meritorious Service Medal.

2.  The applicant states that this is a retirement award and the award was not completed until after she out-processed from Fort Hood, Texas. 

3.  The applicant provides orders for the Meritorious Service Medal and a copy of her DD Form 214 in support of her application.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 3 March 1988.  She remained on active duty through continuous reenlistments until she retired on 31 May 2008 in the rank of sergeant first class. 

2.  The applicant’s DD Form 214 shows, among other awards, two awards of the Meritorious Service Medal.

3.  Section VIII (Awards and Decorations) on the applicant’s Enlisted Record Brief, dated 7 September 2007, shows entitlement to two awards of the Meritorious Service Medal.

4.  Headquarters, III Corps Permanent Order A-056-3, dated 25 February 2008, shows the applicant received the Meritorious Service Medal for exceptionally meritorious service during the period 1 May 1998 to 30 April 2008.

DISCUSSION AND CONCLUSIONS:

The applicant’s Enlisted Record Brief, dated September 2007, shows entitlement to two awards of the Meritorious Service Medal in the awards and decorations section.  Orders, dated February 2008, show the applicant received a third award of the Meritorious Service Medal.  Therefore, her DD Form 214 should be corrected to show three awards of the Meritorious Service Medal.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending her DD Form 214 to show three awards of the Meritorious Service Medal.
