THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show his deployment in Kuwait. 2. The applicant states that he was deployed to Kuwait from 1 February 2003 to 30 April 2003. 3. The applicant provides a Veteran Military History document (Veterans Information Solution), printed on 28 October 2008; an Enlisted Record Brief; and a copy of his DD Form 214 in support of his application. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 18 March 2002 and trained in military occupational specialty (MOS) 92R (parachute rigger). On 30 June 2006, he was released from active duty and placed on the Temporary Disability Retired List the following day. 2. Item 12f (Foreign Service) on the applicant’s DD Form 214 shows the entry “0000 00 00” and item 18 (Remarks) does not show any deployments. 3. In support of his claim, the applicant provided a Veteran Military History document which shows, in pertinent part, that he received hostile fire/imminent danger (HFP/IDP) pay from 1 February 2003 to 30 April 2003 for service in Kuwait. 4. Records at the Defense Finance and Accounting Service (DFAS) also show the applicant served in Kuwait from 1 February 2003 to 30 April 2003 (a total of 3 months). 5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. It states that, for Item 12f, from the Soldier's records, enter the total amount of foreign service completed during the period covered in Item 12c (Net Active Service This Period). The regulation also states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be entered in item 18. DISCUSSION AND CONCLUSIONS: The Veteran Military History document provided by the applicant and the DFAS email which show the applicant served in Kuwait from 1 February 2003 to 30 April 2003 are accepted as sufficient evidence in which to amend items 12f and 18 of the applicant’s DD Form 214.
