Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Armed Forces Service Medal (AFSM). 2. The applicant states, in effect, that her DD Form 214 is missing the AFSM. She also states that she did not receive the Memorandum for Record (MFR) awarding the AFSM until April 2007 after she had been discharged. 3. In support of her application, the applicant provides copies of the MFR and her DD Form 214. CONSIDERATION OF EVIDENCE: 1. The applicant's record shows she enlisted in the Regular Army, in pay grade E-1, on 28 October 1994. 2. The applicant provided a copy of an MFR, dated 24 March 2006, Subject: Award of the AFSM, recommending this award for those Soldiers serving with Headquarters, Department of the Army, Office of the Deputy Chief of Staff, G-3/5/7, Army Operations Center at the Pentagon for service during Hurricanes Katrina and Rita Disaster Relief for services on or after 27 August 2005 for a period of not less than 30 consecutive or 60 non-consecutive days. The applicant's name was listed as one of those Soldiers. 3. The applicant served continuously until she was honorably discharged on 10 December 2006. Her DD Form 214 shows she was awarded the Army Commendation Medal (2nd Award), the Air Force Outstanding Unit Award, the Army Superior Unit Award, the Army Good Conduct Medal (3rd Award), the National Defense Service Medal (2nd Award), the Global War on Terrorism Service Medal, the Overseas Service Ribbon, and the Army Staff Identification Badge. The Armed Forces Service Medal is not listed. DISCUSSION AND CONCLUSIONS: 1. The evidence shows the applicant was awarded the Armed Forces Service Medal in recognition of her service during Hurricanes Katrina and Rita Disaster Relief. Therefore, there is sufficient evidence on which to base correction of her DD Form 214 to show this award. 2. In view of the foregoing, the applicant’s records should be corrected as recommended below.
