THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of his Certificate of Release or Discharge from Active Duty (DD Form 214) to show his foreign service in Iraq. 2. The applicant states that Item 12 (Record of Service) of his DD Form 214 does not show his foreign service. 3. The applicant provides copies of his DD Form 214 and Post-Deployment Health Reassessment (DD Form 2900). CONSIDERATION OF EVIDENCE: 1. On 17 December 2003, the applicant was appointed a second lieutenant in the United States Army Reserve. On 12 March 2004, he was ordered to active duty. 2. On 12 September 2005, the applicant was promoted to first lieutenant. 3. The applicant's Officer Evaluation Report (OER) for the period from 25 March 2005 through 24 March 2006 shows he was assigned as an Assistant Squadron Plans Officer in support of Iraqi Freedom. As such, he was a primary trainer to the Iraqi Army. 4. The applicant's OER for the period from 25 March 2006 through 14 December 2006 shows he was assigned as an Assistant Intelligence Officer in support of Iraqi Freedom. As such, he performed well in combat in an extremely complex and contentious area in Muqdadiya, Iraq. 5. Permanent Orders 122-182D, 101st Airborne Division, dated 2 May 2006, awarded the applicant the Combat Action Badge for engaging the enemy on 27 March 2006. 6. On 21 December 2006, the applicant was released from active duty and transferred to the United States Army Reserve Control Group (Reinforcement). He had completed 2 years, 9 months, and 10 days of creditable active duty. 7. Item 12f (Foreign Service) of the applicant's DD Form 214 shows no overseas service. 8. A DD Form 2900, dated 18 December 2006, shows the applicant served in Iraq from 26 September 2005 through 7 September 2006, a period of 11 months and 12 days. 9. Army Regulation 635-5 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214.  It provides, in pertinent part, for an entry in block 12f (Foreign Service) of the total amount of foreign service completed during the current period of active duty. It further provides that Item 18 (Remarks) will contain the following entry for Regular Army Soldiers: "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)." DISCUSSION AND CONCLUSIONS: 1. The applicant contends that his DD Form 214 should be corrected to show his service in Iraq. 2. The available evidence clearly shows the applicant served in Iraq from 26 September 2005 through 7 September 2006. 3. In view of the above, the applicant's DD Form 214 should be corrected to show in Item 12f a period of 11 months and 12 days of foreign service, and in Item 18 (Remarks), "SERVICE IN IRAQ FROM 20050926 - 20060907.
