THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show award of the Army Achievement Medal (AAM) (6th Award), the Meritorious Service Medal (MSM), and the Meritorious Unit Commendation (MUC). The applicant also requests that one of the double entries made for the Driver and Mechanic Badge with Driver-Tracked Vehicles(s) Clasp be removed from his DD Form 214. 2. The applicant states that the retirement counselor did not annotate these awards on his DD Form 214. 3. The applicant provides copies of his AAM orders; a copy of his MSM orders and certificate; a copy of his MUC orders; a copy of his DD Form 214; and a copy of his Enlisted Record Brief. CONSIDERATION OF EVIDENCE: 1. The applicant served in the Regular Army from 28 October 1987 through 15 April 2008. He was trained in, awarded, and served in military occupational specialty (MOS) 21B (Combat Engineer). He attained the grade of staff sergeant/E-6. The applicant served in Iraq from 16 November – 17 December 2003 and from 12 March 2004 – 11 March 2005. He was placed on the Temporary Duty Retirement List (TDRL), rated as 60 percent disabled. 2. The applicant's DD Form 214 already shows award of the MUC. Therefore, this award will not be discussed again in these proceedings. 3. The applicant provided copies of his AAM permanent orders and award certificates showing that he was awarded the AAM (6th Award). The applicant's Enlisted Records Brief shows he was awarded the AAM (6th Award). These awards are not shown on his DD Form 214. 4. Permanent Orders Number 164-017, Headquarters, 1st Cavalry Division, Fort Hood, Texas, dated 12 June 2008, awarded the applicant the MSM which is not shown on his DD Form 214. 5. The applicant's DD Form 214 shows two entries for the Driver and Mechanic Badge with Driver-Tracked Vehicles(s) Clasp. 6. Army Regulation 635-5 (Separation Documents) prescribes policies and procedures regarding separation documents (DD Form 214/5). Item 13 of the DD Form 214 lists awards and decorations for all periods of service in the priority sequence specified in AR 600-8-22 (Military Awards). Each entry is verified by the individual's service record. DISCUSSION AND CONCLUSIONS: 1. Permanent orders awarded the applicant the AAM (6th Award). Therefore, he is entitled to have his record corrected to show these awards. 2. Permanent orders awarded the applicant the MSM. Therefore, he is entitled to have his record corrected to show this award. 3. The Driver and Mechanic Badge with Driver-Tracked Vehicles(s) Clasp is entered twice on the applicant's DD Form 214. Therefore, he is entitled to have one entry removed.
