THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show award of the Combat Action Badge (CAB) and the Bronze Star Medal (BSM). 2. The applicant states, in effect, that these awards were not added to his DD Form 214 prior to his discharge. 3. The applicant provides a copy of his DD Form 214; a copy of the order awarding him the CAB; and a copy of his BSM Citation and associated documents. CONSIDERATION OF EVIDENCE: 1. The applicant is a lieutenant colonel in the U.S. Army Reserve. He was ordered to active duty in support of Operation Enduring Freedom from 18 March 2005 through 20 April 2006. 2. Permanent Orders 106-021, Department of the Army (DA), Combined Security Transition Command-Afghanistan, Kabul, Afghanistan, dated 16 April 2006, awarded the applicant the CAB (First Award) for actively engaging or being engaged by the enemy on 23 December 2005. 3. Permanent Orders 127-001, DA, Combined Security Transition Command-Afghanistan, Kabul, Afghanistan, dated 17 April 2006, awarded the applicant the BSM for exceptionally meritorious service while deployed in support of Operation Enduring Freedom. 4. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the applicant’s DD Form 214 does not show award of the CAB or BSM. DISCUSSION AND CONCLUSIONS: 1. Permanent Orders awarded the applicant the CAB. Therefore, he is entitled to have his records corrected to show this award. 2. Permanent Orders awarded the applicant the BSM. Therefore, he is entitled to have his records corrected to show this award.
