2. The applicant requests correction of his military records to reflect award of two Army Commendation Medals. 3. Records available to the Board indicate the applicant served on active duty between 11 April 1990 and 10 April 1994. He was awarded his first Army Commendation Medal while assigned to Germany in 1991 and his second while assigned to Fort Lewis, Washington. Although both awards are confirmed in orders neither was recorded on his DD Form 214 at the time of his separation from active duty in 1994. CONCLUSIONS: 1. The evidence confirms the applicant was awarded two Army Commendation Medals while on active duty between 1990 and 1994 although neither award is reflected on his DD Form 214. 2. In view of the foregoing, it would be appropriate to correct the applicant’s records as recommended below. RECOMMENDATION: That all of the Department of the Army records related to this case be corrected by showing that the individual concerned was awarded two Army Commendation Medals.
