2.  In effect, the applicant requests that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show that she was on active duty from 
7 September 1990 through 21 March 1991, inclusive.  She states that her DD Form 214 does not show that she served on active duty from 7 September through 20 November 1990.  

3.  The applicant was a Reserve officer assigned to the 50th General Hospital in Seattle Washington.  On 23 August 1990 The Total Army Personnel Command in St. Louis ordered the applicant to active duty in support of Operation Desert Storm with a reporting date to Madigan Army Medical Center of 7 September 1990.  

4.  On 17 November 1990 Headquarters, Sixth Army ordered all unit personnel of the 50th General Hospital to active duty with a reporting date to their home station of 
21 November 1990.  On 17 November 1990 the 124th Army Reserve Command directed that the applicant and other personnel in her unit, by name, report to the 50th General Hospital on 21 November 1990, and report to Fort Lewis, Washington, the mobilization station, on 24 November 1990.

5.  The applicant served on active duty in support of Operation Desert Shield/Desert Storm, serving in southwest Asia from 12 January to 13 March 1991.  She was released from active duty on 21 March 1991.

6.  The applicant’s DD Form 214 shows that she was on active duty from 21 November 1990 through 21 March 1991 for a total of 4 months and 1 day.

7.  Information obtained from the Defense Finance and Accounting Service in Indianapolis is that the applicant was on active duty from 7 September 1990 through 21 March 1991, as claimed.

CONCLUSIONS:

1.  The applicant served on active duty from 
7 September 1990 through 21 March 1991.  Her DD Form 214 should be changed accordingly. 

2.  In view of the foregoing findings and conclusion, it would be appropriate to correct the applicant’s record as recommended below.

RECOMMENDATION:

That all of the Department of the Army records related to this case be corrected by showing that the individual concerned was on active duty from 7 September 1990 through 21 March 1991.
