2. The applicant requests in effect, that his DD Form 214 (Certificate of Release or Discharge from Active Duty), be corrected to show that he entered active duty on 15 October 1975. 3. He was born on 22 June 1957. He completed 12 years of formal education. On 15 October 1975, he enlisted in the Regular Army for 3 years. His Armed Forces Qualification Test score was 21 (Category IV). He completed the required training and was awarded military occupational specialty 11B10 (Infantryman). His service, thereafter was continuous through reenlistments and extensions. The highest grade he achieved was pay grade E-5. 4. On 30 January 1992, the applicant voluntarily requested a discharge under the provisions of Army Regulation 635-200, chapter 16, paragraph 8, (Voluntary Enlisted Incentive Separation Program) On 31 January 1992, the applicant’s request for discharge was approved with a separation date of 1 July 1992. 5. On 1 July 1992, the applicant was honorably discharged, in pay grade E-5, under the provisions of Army Regulation 635-200, chapter 16, paragraph 8 (Enlisted Voluntary Early Transition Program). His DD Form 214, indicated that he had a total of 16 years, 8 months and 16 days of honorable active service. He was authorized the Army Commendation Medal (second oakleaf clusters), the Army Achievement Medal (third oakleaf clusters), the Good Conduct Medal (fifth award), the National Defense Service Medal, the Noncommissioned Officer Professional Development Ribbon (second award), the Army Service Ribbon and the Overseas Service Ribbon. CONCLUSIONS: 1. The evidence of record shows that the applicant entered into the Regular Army on 15 October 1975. 2. In view of the foregoing, it would be appropriate to correct the records as recommended below. RECOMMENDATION: 1. That all of the Department of the Army records related to this case be corrected by showing that the individual concerned enlisted in the Army on 15 October 1975. 2.
