The applicant requests that block 12h of his DD Form 214, effective date of pay grade, be corrected from 1 April 1992, the date he entered on active duty, to 5 August 1991, the date he enlisted in the USAR. 3. The applicant states that an error was made in the preparation of his DD Form 214. 4. The applicant's military records show that he enlisted in the USAR with no prior service on 5 August 1991 in pay grade E-3. 5. He entered on active duty on 1 April 1992, was awarded the military occupational specialty of water treatment specialist, and was released from active duty and returned to his USAR unit on 24 August 1992. 6. The DD Form 214 he was issued, block 12h, effective date of pay grade, lists 1 April 1992. CONCLUSIONS: 1. It appears evident that the separation and transfer point which prepared the applicant’s DD Form 214 did not have his USAR records and were required to use the applicant’s date of entry on active duty as his date of rank in the absence of those records. 2. In view of the foregoing, the applicant’s records should be corrected as recommended below. RECOMMENDATION: That all of the Department of the Army records related to this case be corrected by showing item 12h of the DD Form 214 of the individual concerned, effective date of pay grade, as 5 August 1991.
