The applicant requests correction of her separation document (DD Form 214) to reflect the correct date and time served during her call to active duty in support of Operation Desert Shield/Storm. 3. The applicant states that her DD Form 214 incorrectly indicates that she entered active duty on 1 February 1991 and that she served 5 months and 9 days of active during that period. She further states that she entered active duty on 8 January 1991 and provides a copy of her order to active duty in support of her application. 4. The applicant’s DD Form 214 indicates that the applicant entered active duty on 1 February 1991 and served for a period of 5 months and 9 days before being released from active duty on 10 July 1991 and transferred back to her USAR unit in Tampa, Florida. 5. The applicant’s military records show that she received an officer evaluation report (OER) at Fort Rucker, Alabama for the period 11 January 1991 through 7 July 1991, evaluating her as a medical surgical nurse in the rank of major. The OER was rendered upon her release from active duty. 6. The orders provided by the applicant indicate that her unit was ordered to active duty in support of Operation Desert Shield on 8 January 1991, in Tampa, Florida, with a report date to Fort Rucker on 11 January 1991. The orders specified that the unit’s members were ordered to active duty at the home station (Tampa, Florida). CONCLUSIONS: 1. The applicant’s DD Form 214 incorrectly reflects the date she entered active duty as 1 February 1991. The applicant was ordered to active duty on 8 January 1991 at her station and reported to Fort Rucker on 11 January 1991. 2. Although the applicant’s OER reflects that she was rated from 11 January 1991 through 10 July 1991, it is reasonable to presume that she complied with her orders and reported with her unit as directed by her orders. 3. Therefore, the applicant’s DD Form 214 dated 10 July 1991 should be corrected to reflect that she entered active duty on 8 January 1991 and was credited with the appropriate amount of active service. 4. In view of the foregoing, it would be appropriate to correct the applicant’s records as recommended below. RECOMMENDATION: That all of the Department of the Army records related to this case be corrected by showing that the date the individual concerned entered active duty was 8 January 1991 instead of 1 February 1991, and that she be given credit for the appropriate amount of active service.
