In effect, the applicant requests that his induction date of 4 November 1965 and his separation date of 13 August 1966 on his DD Form 214 (Report of Transfer or Discharge) be changed to 3 November 1963 and 15 August 1965, respectively. 3. The applicant’s record of induction (DD Form 47) shows that he was inducted into the Army on 4 November 1964 at the Armed Forces Induction Station at Columbus, Ohio, and ordered to report to the Army reception station at Fort Knox, Kentucky on 5 November. The applicant underwent a medical examination at the Armed Forces Examining Station at Fort Hayes on 4 November 1964. The applicant signed a statement acknowledging that he had a 6 year service obligation on 4 November, and orders dated 4 November 1964 assigning the applicant to Fort Knox indicate that he was inducted on that date. 4. Fort Knox orders of 7 December 1964 assigning the applicant to Fort Leonard Wood, Missouri for advanced individual training show the applicant’s basic pay entry date as 4 November 1964. On 5 March 1965 the applicant was assigned to Fort Benning, Georgia. His entry date is shown as 4 November 1964. In July 1965 he was assigned to the 8th Engineer Battalion of the 1st Cavalry Division and moved with that unit to Vietnam, returning in August 1966. He was promoted to pay grade E-4 on 28 May 1966 while in Vietnam. 5. The applicant was assigned to the 8th Engineer Battalion in Vietnam when it was awarded the Presidential Unit Citation, the Meritorious Unit Commendation, and the Republic of Vietnam Gallantry Cross with Palm Unit Citation. He participated in three campaigns and should have been awarded the Vietnam Service Medal with three bronze service stars. Additionally, he served in Vietnam for more than six months, and should have been awarded the Republic of Vietnam Campaign Medal. None of the aforementioned awards are shown on the applicant’s DD Form 214. 6. The applicant received a separation physical examination on 12 August 1966. He completed and signed the report of medical history for that examination. Orders published by the Army personnel center at Oakland, California on 13 August 1966 show that the applicant was released from active duty on that date and transferred to the Reserves for completion of his remaining service obligation. CONCLUSIONS: 1. The applicant was inducted into the Army on 4 November 1964. His DD Form 214 should be changed accordingly. 2. The applicant was released from active duty on 13 August 1966. This date, the effective date of his separation as shown on his DD Form 214, is correct. 3. The applicant is entitled to award of the Presidential Unit Citation, the Meritorious Unit Commendation, the Republic of Vietnam Gallantry Cross with Palm Unit Citation, the Vietnam Service Medal with three bronze service stars, and the Republic of Vietnam Campaign Medal. These awards should be shown on his DD Form 214. 4. In view of the foregoing findings and conclusions, it would be appropriate to correct the applicant’s records as recommended below. RECOMMENDATION: 1. That all of the Department of the Army records related to this case be corrected by showing that the individual concerned was inducted into the Army on 4 November 1964; that he was awarded the Presidential Unit Citation, the Meritorious Unit Commendation, the Republic of Vietnam Gallantry Cross with Palm Unit Citation, the Vietnam Service Medal with three bronze service stars, and the Republic of Vietnam Campaign Medal. 2.
