RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS IN THE MATTER OF: DOCKET NUMBER: BC-2011-03360 COUNSEL: NO HEARING DESIRED: NO _________________________________________________________________ APPLICANT REQUESTS THAT: His records be corrected to reflect entitlement to award of the Air Force Commendation Medal (AFCM). _________________________________________________________________ APPLICANT CONTENDS THAT: He was given the certificate and citation for the AFCM; however he never received the actual medal. Also, the special order was either lost or never forwarded to the Air Force Personnel Center (AFPC). In support of his request, the applicant provides a copy of his DD Form 214, Armed Forces of the United States Report of Transfer or Discharge and AFCM citation and certificate. The applicant's complete submission, with attachments, is at Exhibit A. _________________________________________________________________ STATEMENT OF FACTS: The applicant served on active duty in the Air Force from 7 Dec 65 to 17 Apr 69. _________________________________________________________________ AIR FORCE EVALUATION: AFPC/DPSIDR recommends denial. The applicant provided an unsigned certificate and citation for his service with the 19 Tactical Air Support Squadron, Bien Hoa Air Base, Republic of Vietnam; however, no special order could be located verifying the applicant was submitted for, or ever awarded the AFCM. The complete DPSIDR evaluation, with attachment, is at Exhibit C. _________________________________________________________________ APPLICANT'S REVIEW OF AIR FORCE EVALUATION: In response to the Air Force advisory opinion, the applicant provided a more legible copy of the signed AFCM certificate with his Congressional Inquiry (Exhibit E). _________________________________________________________________ THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was not timely filed; however, it is in the interest of justice to excuse the failure to timely file. 3. Sufficient relevant evidence has been presented to demonstrate the existence of an error or injustice concerning the Air Force Commendation Medal (AFCM). In this respect, we note the AFCM certificate and citation provided by the applicant reflects the decoration was awarded for meritorious service from 20 April 1968 to 10 April 1969. Although the applicant has not provided a copy of the order awarding the AFCM, we find the evidence presented sufficient to resolve any doubt in favor of the applicant. Therefore, we recommend his records be corrected as indicated below. _________________________________________________________________ THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to APPLICANT, be corrected to show that he was awarded the Air Force Commendation Medal, for the period of 20 April 1968 to 10 April 1969. _________________________________________________________________ The following members of the Board considered Docket Number BC- 2011-03360 in Executive Session on 20 Mar 12, under the provisions of AFI 36-2603: Panel Chair Member Member All members voted to correct the records, as recommended.
